摘　　要：报告1例伴毛囊黏蛋白病的蕈样肉芽肿。患者男,37岁。面部、躯干及四肢红色斑片和斑块2个月。皮肤科检查:面部、躯干及四肢散在境界清楚,形态不规则的红斑,红斑上有少量粘着性鳞屑。皮损组织病理:真皮毛囊内水肿,其内较多黏蛋白沉积,阿新蓝染色阳性;毛囊周围可见较多淋巴细胞和嗜酸性粒细胞浸润,部分小淋巴细胞轻度不规则;免疫组化:CD3、CD4及CD5(+),CD8及CD20(-),诊断:伴毛囊黏蛋白病的蕈样肉芽肿。A case of folliculotropic granuloma fungoides with follicular mucinosis is reported. A 37-year-old man presented with infiltrated erythematous patches on face and trunk for 2 months. Dermatologic examination revealed infiltrating erythema with a clear boundary, irregular shape, and a small number of adhesive scales scattered on the face, trunk and limbs. Histopathological examination revealed edema of hair follicle epithelial cells in the dermis with mucinprotein deposition in follicular epithelum, and a positive result for alcian blue staining. Infiltration of massive lymphoid cells and eosinophilic granulocytes was found around the hair follicles, and some small lymphocytes had slightly irregular nuclei. Immunohistochemical staining confirmed that most infiltrated lymphocytes were CD3(+), CD4(+), CD5(+), CD8(-), and CD20(-). The diagnosis of folliculotropic granuloma fungoides with follicular mucinosis was made.
