摘　　要：目的:研究紫杉醇药物涂层球囊(DCB)在治疗自体动静脉内瘘(AVF)及人工血管内瘘(AVG)血管狭窄中的安全性及有效性。方法:选取我院从2018年1月~2019年10月收治的血液透析AVF及AVG血管狭窄患者84例进行研究。将其以信封法随机等分成研究组和对照组。对照组实施经皮普通球囊扩张成形术治疗,研究组则实施经皮DCB扩张成形术治疗。比较两组在治疗效果,治疗前后血管内径以及透析血流量,治疗前后血清单核细胞趋化蛋白-1(MCP-1)、血管内皮生长因子-A(VEGF-A)水平,术后并发症发生情况等方面的差异。研究组术后6个月内瘘通畅率显著高于对照组,而再狭窄率显著低于对照组(均P<0.05)。结果:研究组治疗成功率、临床成功率均高于对照组(均P<0.05)。治疗后研究组血管内径以及透析血流量均高于对照组(均P<0.05)。治疗后研究组血清MCP-1、VEGF-A水平均低于对照组(均P<0.05)。研究组及对照组在穿刺部位血肿、渗血、手背肿胀发生率方面对比均不明显(均P>0.05)。结论:DCB治疗AVF及AVG血管狭窄患者的效果显著,有利于改善患者的血管内径以及透析血流量,安全性较佳,值得临床推广应用。To study the safety and efficacy of paclitaxel coated balloon(DCB)in the treatment of AVF and AVG stenosis.84 patients with AVF and AVG vascular stenosis admitted to our hospital from January 2018 to October 2019 were selected for study.They were randomly divided into study group and control group by envelope method.The control group was treated with percutaneous balloon dilatation,while the study group underwent percutaneous DCB dilatation.The two groups were compared in terms of treatment effect,vascular diameter and hemodialysis blood flow before and after treatment,serum monocyte chemoattractant protein-1(MCP-1),vascular endothelial growth factor-a(VEGF-A)levels before and after treatment,and postoperative complications.The treatment success rate and clinical success rate in the study group were higher than those in the control group(P<0.05).After treatment,the vascular diameter and dialysis blood flow in the study group were higher than those in the control group(P<0.05).After treatment,the serum levels of MCP-1 and VEGF-A in the study group were lower than those in the control group(all P<0.05).There was no significant difference between the study group and the control group in the incidence of hematoma,oozing,and hand back swelling at the puncture site(all P>0.05).In the study group,the patency rate of fistula within 6 months after surgery was significantly higher than that of the control group,while the restenosis rate was significantly lower than that of the control group(all P<0.05).DCB is effective in the treatment of AVF and AVG patients with vascular stenosis,which is conducive to improving the vascular diameter and hemodialysis blood flow in patients with better safety.It is worthy of clinical application.
