摘　　要：目的研究可溶性Neuropilin-1(sNRP-1)对早期宫颈病变可能的诊断和预后评估价值。方法采用ELISA法检测31例早期宫颈鳞状细胞癌(SCC)患者、18例癌前病变(CIN)患者和20例对照组患者血清中sNRP-1的水平,结合患者的分期、淋巴结转移情况、预后等信息,分析sNRP-1对宫颈病变的诊断价值和预后评估价值。结果相比对照组,sNRP-1在SCC组和CIN组均显著升高,但在SCC组和CIN组之间并无显著差异;同时淋巴结阳性的SCC患者相对淋巴结阴性的SCC患者,其sNRP-1水平更高。ROC曲线分析表明sNRP-1对SCC和CIN具备诊断价值,sNRP-1为9.535 ng/mL时,其对应的敏感度为65.31%,特异度为84.21%。高sNRP-1组的SCC患者其无瘤生存期(DFS)较低sNRP-1组的SCC患者更短,差异具有显著的统计学意义(P<0.05)。结论sNRP-1是极具潜力的早期宫颈鳞癌肿瘤标记分子。Objective To investigate diagnostic and prognostic value of soluble Neuropilin-1(sNRP-1)in cervical cancer or cervical intraepithelial neoplasia(CIN).Methods By using ELISA,sNRP-1 was measured in plasma of 49 patients[including 31 cervical squamous cell carcinoma(SCC)and 18 CIN cases]before treatment and 20 controls.Diagnostic value and prognostic value of sNRP-1 for cervical lesions were analyzed in combination of information about patient stage,lymph node metastasis and prognosis.Results Compared with the control group,sNRP-1 was significantly increased in SCC and CIN groups,without significant difference between SCC and CIN groups.sNRP-1 was higher in SCC patients with lymph node metastasis than those without lymph node metastasis.ROC curve analysis showed sNRP-1 had a diagnosis value for SCC and CIN.When sNRP-1 was 9.535 ng/mL,its sensitivity was 65.31%and specificity 84.21%.Importantly,SCC patients with high sNRP-1 had shorter disease-free survival(DFS)than those with low NRP-1.Conclusion sNRP-1 can serve as a valuable biomarker for early SCC.
