质子泵抑制剂联合瑞巴派特与质子泵抑制剂单药治疗内镜下粘膜剥离术后溃疡的Meta分析

摘　　要：目的比较质子泵抑制剂(proton pump inhibitor,PPI)联合瑞巴派特与PPI单药治疗,对内镜下粘膜剥离术(endoscopic submucosal dissection,ESD)后溃疡愈合的疗效。方法计算机检索PubMed、EMbase、EBSCO、Elsevier和Web of Science数据库中的相关研究文献,检索时间从数据库建库至2020年12月31日。由两位研究者使用Jadad量表来评估随机对照试验的质量,并按照纳入、排除标准独立提取数据,采用RevMan 5.2和Stata 12.0软件进行统计学分析。结果共纳入7项研究,811名受试者。PPI联合瑞巴派特治疗ESD术后溃疡的治愈率明显高于PPI单药治疗[OR=2.33,95%CI(1.68,3.27),P<0.001]。联合治疗时,在PPI的种类和剂量方面,更倾向于选择雷贝拉唑20 mg[OR=4.53,95%CI(1.92,10.70),P=0.0006]和泮托拉唑40 mg[OR=1.93,95%CI(1.04,3.60),P=0.04]。结论与PPI单药治疗相比,PPI联合瑞巴派特治疗ESD术后溃疡愈合的效果更佳。Objective To compare the efficacy of proton pump inhibitor(PPI)combined with ribapide and PPI alone in the treatment of ulcer healing rate after endoscopic submucosal dissection(ESD).Methods Computer search of relevant research literature in PubMed,EMbase,EBSCO,Elsevier and Web of Science databases,the search time was from the establishment of the database to December 31,2020.Two researchers used the Jadad scale to evaluate the quality of randomized controlled trials,and independently extracted data according to the inclusion and exclusion criteria,and used RevMan 5.2 and Stata 12.0 software for statistical analysis.Results A total of 7 studies,including 811 subjects,were met the inclusion criteria.Compared to the PPI monotherapy,the healing rate of ESD-induced ulcers was much higher in PPI plus rebamipide combination therapy[OR=2.33,95%CI(1.68,3.27),P<0.001].In terms of the type and dose of PPI,rabeprazole 20 mg[OR=4.53,95%CI(1.92,10.70),P=0.0006]and pantoprazole 40 mg[OR=1.93,95%CI(1.04,3.60),P=0.04]were more preferred when choosing PPI for combination treatment.Conclusion Combination therapy is more efficient than PPI monotherapy in healing ESD-induced gastric ulcers.
