摘  要:目的探讨四维剂量评估与传统三维评估非小细胞肺癌SBRT放疗计划的剂量学差异。方法将患者的三维SBRT计划的剂量分别映射到不同呼吸时相的图像上,计算实际剂量并得到不同时相下的剂量分布。将不同呼吸时相下的剂量分布分别形变映射至平均密度投影图像上,并将剂量平均加权得到四维剂量结果。比较分析三维剂量和四维剂量的评估结果。结果3D和4D剂量计算评估比较,除肺的V_(1000)、V_(2000)、心脏的D_(mean)及R_(50%)剂量学差异无统计学意义(P>0.05)外,其余靶区和危及器官的剂量均有统计学意义(P<0.05),靶区剂量偏差范围为-3.62%~1.24%,危及器官偏差范围-2.14%~0.99%。结论使用四维剂量评估放疗计划可以实现更加精确的放疗。Objective To investigate the dosimetry difference between four dimensional dose assessment and traditional three-dimensional dose assessment of SBRT radiotherapy for non—small cell lung cancer.Methods The dose of the three-dimensional SBRT plan of the patient was mapped to the images of different respiratory phases,the actual dose was calculated and the dose distribution under different respiratory phases was obtained;the dose distribution under different respiratory phases was deformed and mapped to the average density projection image respectively,and the four-dimensional dose result was obtained by the average dose distribution weight;the results of three-dimensional dose and four-dimensional dose were compared and analyzed by statistical method.Results By comparing the 3D and 4D dose distribution,there was no significant difference in the dose of V1000,V2000 and Dmean of lung,Dmean of heart and R50% between the two groups(P>0.05),but there was no difference in R50% between the does of other target area and the organ at risk(P<0.05).The dose deviation ranged from -3.62% to 1.24% in all target and from -2.14% to 0.99% in organs at risk.Conclusion More accurate radiotherapy can be achieved by using four-dimensional dose assessment radiotherapy plans.
