摘 要:目的探讨融合转录本E1/CCAT1在人乳头瘤病毒(HPV)18感染的宫颈组织和上皮细胞中的表达及临床意义。方法选取2016年2月至2020年3月浙江省中医院收治的HPV18感染宫颈癌前病变及宫颈癌患者共62例和宫颈癌筛查者105例作为研究对象。收集所有患者经手术或活检获取的宫颈组织和筛查者宫颈脱落上皮细胞标本,培养Hda细胞。以非HPV18感染或因良性疾病切除的宫颈组织作为对照,通过实时荧光定量PCR(RT-qPCR)法检测HeLa细胞株及收集样本的融合转录本E1/CCAT1和C-MYC的表达。结果在HPV18阳性的部分宫颈组织标本和HeLa细胞株中均发现E1/CCAT1(E1_1489084/Chrom8_128,231,211)的表达。E1/CCAT1和C-MYC的相对表达量均随着宫颈分级程度加重而逐级升高,且E1/CCAT1在宫颈病变组织中的相对表达水平与C-MYC呈正相关(P<0.01,r=0.8862)。105例HPV18阳性的宫颈脱落上皮细胞中有10例检测出E1/CCAT1融合转录本,后续随访经阴道镜活检为低或高度上皮内瘤变患者。结论HPV18可能通过与宿主细胞转录融合形成E1/CCAT1转录本参与宫颈癌的病变过程,有望成为宫颈癌筛查的新标志物。Objective To explore the expression and clinical significance of fusion transcript E1/CCAT1 in HPV 18 infected cervical tissues and epithelial cells.Methods 62 cases of cervical precancerous lesions and cervical cancer patients infected with HPV18 and 105 cases of cervical cancer screening in Zhejiang Provincial Hospital of Traditional Chinese Medicine from February 2016 to March 2020 were selected as the research objects.Cervical surgery or biopsy tissues of all patients and cervical exfoliated epithelial cell samples of screening population were collected and Hela celk were cultured.Using non-HPV18 infected or excised cervical tissues due to benign diseases as controls,real-time fluorescent quantitative PCR(RT-qPCR)was used to detect the expression of E1/CCAT1 and C-MYC in HeLa cell lines and the fusion transcripts of these samples.Results The expression of E1/CCAT1(E1_1489084/Chrom8_128,231,211)was found in someHPV18 positive cervical tissues and HeLa cell lines.The relative expression levels of E1/CCAT1 and C-MYC increasedgradually with the progression of cervical grading,and the relative expression levels of E1/CCAT1 in cervical lesions were positively correlated with C-MYC(P<0.01,r=0.8862).E1/CCAT1 fusion transcripts were detected in 10 of 105 HPV 18-positive exfoliated cervical epithelial cells,and patients with low or high intraepithelial neoplasia were identified by colposcopic biopsy after follow-up.Conclusion HPV 18 may be involved in the pathological process of cervical cancer through transcriptional fusion with host cells to form El/CCAT1,and is expected to be a new marker for cervical cancer screening.
