中晚期原发性肝癌TACE术后疗效评价标准的比较

摘　　要：目的探讨RECIST标准与mRECIST标准评估中晚期原发性肝癌TACE术后疗效的应用价值。方法回顾性收集75例中晚期原发性肝癌患者TACE治疗前1周、治疗后3个月的螺旋CT动态增强扫描图像资料,分别依据mRECIST标准和RECIST标准进行疗效评估,比较两种标准评价结果的一致性。结果TACE治疗后3个月,75例患者按照RECIST标准进行疗效评价,完全缓解(CR)0例,部分缓解(PR)15例,疾病稳定(SD)41例,客观有效率(RR)为20.00%,疾病控制率(DCR)为74.67%;按照mRECIST标准评价,CR为8例,PR为31例,SD为27例,RR为52.00%,DCR为88.00%。两种标准的评价结果差异具有统计学意义(P<0.05)。结论mRECIST标准强调对靶病灶“存活肿瘤”的测量,能够对中晚期原发性肝癌TACE术后疗效作出更加客观、精确的评估。Objective To explore the application value of RECIST criterion and mP^ECIST criterion in evaluating the postoperarive curative effect of transcatheter arterial chemoembolizarion(TACE)in advanced primary liver cancer.Methods Retrospectively collect the spiral CT dynamic enhancement scan image data of 75 patients with intermediate and advanced primary liver cancer 1 week before TACE treatment and 3 months after treatment.The curative effect was separately evaluated by mRECIST and RECIST criterion and the consistency of the evaluation results by the two criterion was investigated.Results Three months after TACE treatment,75 patients were evaluated for efficacy according to RECIST criterion,0 patient had complete response(CR),15 had partial response(PR),and 41 had stable disease(SD),the objective response rate(RR)was 20.00%,the disease control rate(DCR)was 74.67%.Under mRECIST criterion,8 patients had CR,31 had PR,and 27 had SD,RR was 52.00%,DCR was 88.00%.The difference between the efficacy assessment results of mRECIST and RECIST criterion was statistically significant(P<0.05).Conclusion The mRECIST criterion emphasizes the measurement of viable tumors of target lesions.The evaluation of mRECIST criterion for TACE therapy efficacy in treating advanced primary liver is more objective and reliable.
