翼点入路手术联合尼莫地平治疗前循环出血性颅内动脉瘤对照观察

摘　　要：目的探讨翼点入路手术联合尼莫地平治疗前循环出血性颅内动脉瘤的临床疗效。方法将92例前循环出血性颅内动脉瘤患者按随机数字表法分为两组,各46例。两组均采取翼点入路显微手术,研究组在此基础上联合尼莫地平治疗。比较手术前后两组血清神经元特异性烯醇化酶水平、S100β水平、大脑中动脉血流速度及脑血管痉挛发生率。结果术后两组血清神经元特异性烯醇化酶、S100β水平较术前显著升高(P<0.01),研究组显著高于对照组(P<0.01),两组大脑中动脉血流速度较术前显著降低(P<0.01),但组间比较差异无统计学意义(P>0.05)。研究组脑血管痉挛发生率显著低于对照组(P<0.05)。结论翼点入路显微手术联合尼莫地平可提高前循环出血性颅内动脉瘤患者血清神经元特异性烯醇化酶及S100β水平,降低大脑中动脉血流速度及术后脑血管痉挛发生风险。Objective To explore the clinical efficacy of pterional approach combined with nimodipine in the treatment of anterior circulation hemorrhagic intracranial aneurysms.Methods A total of 92 patients with anterior circulation hemorrhagic intracranial aneurysms were divided into two groups according to random number table method,with 46 cases in each group.Both groups underwent pterional approach microsurgery.The study group added nimodipine on the above basis.The serum neuron specific enolase(NSE),S100βlevels,blood flow velocity of middle cerebral artery,and the incidence of cerebral vasospasm were compared between the two groups before and after surgery.Results The serum levels of NSE and S100βin the two groups were significantly higher than those before operation(P<0.01),and the study group was significantly higher than the control group(P<0.01).The blood flow velocity of middle cerebral artery in the two groups was significantly lower than that before operation(P<0.01).However,there was no significant difference between the groups(P>0.05).The incidence of cerebral vasospasm in the study group was significantly lower than that in the control group(P<0.05).Conclusions Pterional approach microsurgery combined with nimodipine can increase serum NSE and S100βlevels in patients with anterior circulation hemorrhagic intracranial aneurysms,reduce blood flow velocity of middle cerebral artery,and reduce the risk of postoperative cerebral vasospasm.
