白内障超声乳化联合Mori Upright镜直视下分离房角治疗原发性闭角型青光眼的疗效分析

摘　　要：目的分析和探讨白内障超声乳化联合Mori Upright房角镜直视下分离房角治疗原发性闭角型青光眼的疗效。方方法在2019年1—12月于兰州大学第二医院接受治疗的原发性闭角型青光眼患者中,随机收集60例患者(60只眼)作为本研究的研究对象和主体,将其按照"动态随机分组法"分为对照组和观察组,每组30例(30只眼)。对照组采用超声乳化联合传统前房角分离术,观察组采用超声乳化联合Mori Upright房角镜直视下前房角分离术,对比2组患者术后的最佳矫正视力(BCVA)、眼压(IOP)、前房深度(ACD)、角膜内皮细胞密度(ECD)、术后并发症发生率。结果观察组BCVA、ACD、ECD高于对照组(P<0.05),IOP以及术后并发症发生率低于对照组(P<0.05)。结论超声乳化联合Mori Upright房角镜下分离房角治疗原发性闭角型青光眼的疗效显著,安全性高,适合在临床开展和普及。Objective To observe and compare the clinical efficacy of phacoemulsification and intraocular lens implantation combined with Mori Upright of goniosynechialysis in the treatment of primary angle-closured glaucoma. Method 60 cases(60 eyes)of patients diagnosed with primary angle-closure glaucoma in the Second Hospital of Lanzhou University from January 2019 to December 2019 were selected and randomly divided into the control group and the observation group according to dynamic random grouping, with 30 patients(30 eyes) in each group. The control group was treated with phacoemulsification combined with traditional anterior chamber angle separation, and the observation group treated with phacoemulsification combined with Mori Upright of goniosynechialysis. The best corrected visual acuity(BCVA), intraocular pressure(IOP), anterior chamber depth(ACD), the changes of corneal endothelial cell density(ECD), and postoperative reaction rate after operation were compared between the two groups. Results After operation, the BCVA, ACD and the ECD of the observation group was significantly higher than that of the control group(P <0.05). Furthermore, the IOP and postoperative reaction rate of the observation group were significantly lower than that of the control group(P < 0.05). Conclusion The significant effect and safety of the phacoemulsification combined with Mori Upright of goniosynechialysis in the treatment of primary angle-closure glaucoma were obvious and it is suitable for clinical development and application.
