摘　　要：目的:通过参与1例食管癌并发乳糜腹患者的药学会诊,探讨营养药师在营养支持治疗过程的药学实践。方法:营养药师作为营养支持工作小组的成员,根据患者临床症状、生化检查结果,结合文献资料、指南等相关理论知识,以会诊的方式协助临床为患者提供个体化营养支持治疗方案,同时进行全程药学监护与管理。结果:经过动态调整营养支持治疗方案、全程药学服务,该患者腹腔引流乳糜液消失,最终康复出院。结论:临床营养支持治疗需个体化给药,药师需掌握临床营养相关基础知识、基本原则、科学制定个体化治疗方案、全程提供药学监护,达到提高临床疗效,保障用药安全、合理,改善临床结局的目的。Objective:To explore the pharmaceutical practice of nutritional pharmacists in the process of nutritional support treatment by participating in the pharmaceutical consultation of a patient with esophageal cancer complicated with chyloabdomen.Methods:As a member of the nutrition support team,nutritional pharmacists assisted the clinic in the form of consultation according to the patients’ clinical symptoms,biochemical examination results,combined with literature,guidelines and other relevant theoretical knowledge,provided individualized nutritional support treatment schemes for patients,and carried out the whole process pharmaceutical care and management.Results:After dynamically adjusting the nutritional support treatment scheme and the whole process pharmaceutical care,the peritoneal drainage of chylous fluid disappeared,and finally recovered and discharged from the hospital.Conclusion:Clinical nutrition support treatment needs individualized administration.Pharmacists need to master the basic knowledge and basic principles related to clinical nutrition,scientifically formulate individualized treatment plan and provide pharmaceutical care in the whole process,so as to improve the clinical efficacy,ensure the safety and rationality of medication and improve the clinical outcome.
