摘 要:目的:探讨滑膜间充质干细胞在创伤性颞下颌关节强直中的作用。方法:将12只绵羊随机分为2组,每组各6只;实验组和对照组均采用髁突截骨术造成实验动物右侧髁突矢状骨折,实验组保留滑膜组织,对照组术中同时去除部分滑膜组织。分别于术后4周,12周各处死3只动物,进行髁突大体形态学观察,组织病理学染色观察成骨细胞表达及免疫组织化学染色观察滑膜间充质干细胞表面标志物CD44,CD90,CD34,CD45的表达情况。结果:术后12周实验组髁突形态改变明显。组织病理学染色可见实验组成骨细胞表达明显增多。免疫组化结果显示实验组及对照组均可见滑膜间充质干细胞标志物CD44及CD90呈阳性表达,CD34及CD45呈阴性表达,且实验组CD44及CD90表达较对照组明显增加。结论:颞下颌关节骨折后,TMJ滑膜间充质干细胞定向迁移至骨断端之间,使骨折区成骨作用增强,致髁突形态明显改变。Objective:To investigate the role of synovial mesenchymal stem cells in traumatic temporomandibular joint ankylosis.Methods:Twelve sheep were randomly divided into 2 groups, each with 6 sheep.Both in the experimental group and control group, oblique sagittal fracture on the right side by osteotomy was made.And synovial tissue was preserved in the experimental group, synovial tissue was partally removed during the operation in the control group.At the 4 th week and 12 th week after the operation, 3 animals in each group were sacrificed to observe the gross morphology of the condyles.The expression of osteoblasts was observed by histopathological staining, and the expression of surface markers CD44,CD90,CD34 and CD45 of synovial mesenchymal stem cells were observed by immunohistochemical staining.Results:After 12 weeks, the condyle morphology of the experimental group was significantly changed, and the expression of osteocytes was significantly increased by histopathological staining.The results of immunohistochemistry showed positive expression of synovial mesenchymal stem cell markers CD44 and CD90,and negative expression of CD34 and CD45 in both groups, and the expression of CD44 and CD90 in the experimental group was significantly higher than that of the control group.Conclusion:After the fracture of temporomandibular joint, TMJ synovial mesenchymal stem cells were promoted to directional migration between the bone stumps, which may enhanced the osteogenesis of the fracture area and significantly changed the morphology of mandibular condyle.
