摘 要:目的:探讨锥形束CT(cone beam computed tomography, CBCT)评价微种植钉与支抗牙刚性连接辅助隐形矫治远移上颌磨牙的疗效。方法:选取我院2018年1月~2020年1月收治的需采用无托槽隐形矫治技术远移上颌磨牙患者81例为研究对象。于所有患者治疗前(T0)、上颌第一磨牙移动到位(T1)及前牙排齐内收到位(T2)采用CBCT三维测量上颌第一磨牙、上颌第二磨牙以及上颌中切牙位置,比较不同时期上颌第一磨牙、上颌第二磨牙以及上颌中切牙三维方向变化情况。结果:第二磨牙牙根中心点T0、T2期垂直向位置变化比较无明显差异(P>0.05),T1期与T0期及T2期比较垂直向位置变化,差异有统计学意义(P<0.05);第一磨牙近中颊尖点、腭根根尖点、第二前磨牙根尖点、颊尖点在T0、T1、T2 3个时期横向位置变化比较无明显差异(P>0.05);第一磨牙近中颊尖点、腭根根尖点T1、T2期矢状向位置变化比较无明显差异(P>0.05),T0期较T1和T2期矢状向位置变化比较差异均有统计学意义(P<0.05);上颌中切牙切缘中点、根尖点、第二前磨牙颊尖点、根尖点在T0、T1期矢状向位置变化比较无明显差异(P>0.05),T0、T1期较T2期矢状向位置变化比较差异均有统计学意义(P<0.05)。结论:微种植钉与支抗牙刚性连接辅助隐形矫治远移上颌磨牙可有效增强支抗牙的支抗作用,疗效较好,值得临床推广使用。Objective:To analyze the curative effect of rigid connection of micro-screw and anchorage teeth in invisalign on upper molar distalization based on cone beam computed tomography(CBCT).Methods:81 patients receiving invisalign treatment in our hospital from January 2018 to January 2020 were enrolled.CBCT was used to measure the position of maxillary first molars, maxillary second molars and maxillary central incisors before treatment(T0),maxillary first molar distalization(T1)and anterior teeth alignment and adduction(T2).Three dimensional changes of maxillary first molar, maxillary second molar and maxillary central incisor were compared in different periods.Results:There was no significant difference in the vertical position of the center point of the second molars at T0 and T2(P>0.05).The vertical position changes at T1 showed significant difference with those at T0 and T2(P<0.05).There was no significant difference in the transverse position of mesiobuccal apex and palatal apex of the first molar and the apical and buccal apex of the second premolar at T0,T1 and T2(P>0.05).There was no significant difference in sagittal position of mesiobuccal apex and palatal root apex of first molar between T1 and T2(P>0.05).The changes of sagittal position at T0 showed significant difference with those at T1 and T2(P<0.05).There was no significant difference in the sagittal position of the central incisor, buccal and apical points of the second premolar at T0 and T1(P>0.05).The changes of sagittal position at T0 and T1 showed significant difference with those at T2(P<0.05).Conclusion:Rigid connection of micro-screw and anchorage teeth in invisalign on upper molar distalization has achieved good effect, which is worthy of clinical promotion.
