摘  要:目的:比较传统机械去腐后树脂修复和CarisolvⅢ凝胶选择性微创去腐充填后联合超薄瓷贴面修复对前牙深龋缺损治疗的临床疗效。方法:收集观察2019年1月~2020年10月前牙深龋缺损患者65例(77颗患牙)并随机分为两组。微创组(MI组,n=38)采用CarisolvⅢ凝胶化学机械去腐方法以及树脂充填,随后行超薄瓷贴面修复;传统机械组(CM组,n=39)传统机械方法去腐后直接行树脂充填修复。比较两组去腐操作时间,术中疼痛程度数字疼痛评分(NRS),并采用世界牙科联盟(FDI)评价标准评估术后26周、52周和78周修复效果及成功率。结果:MI组去腐操作时间大于CM组,去腐时NRS疼痛程度小于CM组(P<0.05)。贴面修复前MI组FDI评分高于CM组;第26周时FDI评分没有显著差异,但第52周和78周时CM组评分高于MI组(P<0.05)。术后26周和52周时两组成功率无差异,但78周时MI组成功率显著高于CM组(P<0.05)。结论:与传统机械去腐后树脂修复相比,微创去腐技术联合超薄瓷贴面修复前牙深龋缺损虽然去腐时间较长,但能显著减轻操作时疼痛,提高修复成功率,远期疗效和美学效果满意。Objective:To compare the clinical efficacy of conventional mechanical removal of dental caries with composite resin filling versus minimally invasive treatment of caries with ultra-thin ceramic veneers in the repair of deep carious lesions in anterior teeth.Methods:Totally 77 anterior teeth with deep caries lesions in 65 patients from January 2019 to May 2019 were collected and randomly divided into two groups.The minimally invasive(MI)group(n=38)was treated by the Carisolv chemomechanical method(Carisolv Ⅲ)with composite resin filling, followed by bonding with ultra-thin ceramic veneers, and the conventional mechanical(CM)removal group(n=39)was treated by the conventional mechanical method, followed by nanofilled resins.The time required for caries removal was compared between the two groups.Intraoperative pain was evaluated by using the numerical rating scale(NRS).The dental restoration effect and success rate were evaluated at 26 weeks, 52 weeks and 78 weeks postoperatively, using the criteria developed by the FDA World Dental Federation.Results:The time required for removal of caries was significantly longer in the MI group, the pain intensity(NRS)during removal of caries was remarkably milder than that in the CM group(P<0.05).The FDI scores of MI group were significantly higher before veneer repair.At 26 weeks after treatment, there were no significant differences between the two groups.However, the score of CM group was higher at 52 weeks and 78 weeks(P<0.05)after treatment.The success rate was not noticeably different between the two groups at 26 weeks and 52 weeks after treatment.However, at 78 weeks, the success rate in the MI group was markedly higher than that in the CM group(P<0.05).Conclusion:Compared with the conventional mechanical removal of caries with composite resin filling, minimally invasive treatment of caries with ultra-thin ceramic veneers took a longer time for removal of caries in the affected anterior teeth.However, the latter could significantly reduce intraoperative pain, improve th
