摘　　要：目的对比经皮微波消融(MWA)联合;I放射性粒子植入与单纯MWA治疗纵隔恶性肿瘤的近期疗效和安全性。方法回顾性分析67例接受治疗的纵隔恶性肿瘤患者的临床资料,其中36例患者采用单纯MWA治疗(单纯MWA组),31例患者采用MWA联合;I放射性粒子植入治疗(联合治疗组)。根据增强CT对比观察术后3个月两组的疗效。对比观察术后1个月内出血、气胸、胸水等相关并发症发生情况。结果两组患者治疗后1个月内相关并发症发生率差异无统计学意义(P>0.05);治疗后3个月,联合治疗组有效率为83.9%,单纯MWA组为55.6%,差异有统计学意义(P<0.05)。结论与单纯MWA相比,MWA联合;I放射性粒子植入治疗纵隔肿瘤,疗效明显提高,且不会增加相关并发症发生率,值得临床推广应用。Objective To compare the short-term effect and safety of percutaneous microwave ablation(MWA)combined with;I seeds implantation with those of simple MWA in treating mediastinal malignant tumors.Methods The clinical data of 67 patients with mediastinal malignant tumor,who received treatment at the Department of Interventional Radiology,Shenzhen Municipal People’s Hospital of China,were retrospectively analyzed.Of the 67 patients,36 received simple MWA treatment(simple MWA group)and 31 received MWA combined with;I seeds implantation(combination treatment group).Based on the contrastenhanced CT findings,the postoperative 3-month curative effect was compared between the two groups.The procedure-related complications occurring within one month after treatment such as bleeding,pneumothorax,pleural effusion,etc.were recorded.Results No statistically significant difference in the incidence of procedure-related complications occurring within one month after treatment existed between the two groups(P>0.05).Three months after treatment,the effective rate in the combination treatment group was 83.9%and in the simple MWA group was 55.6%,the difference between the two groups was statistically significant(P<0.05).Conclusion Compared with simple MWA in treating mediastinal malignant tumors,WMA combined with;I seeds implantation can significantly improve the curative efficacy,while not increasing the incidence of procedure-related complications.Therefore,this therapy is worthy of clinical promotion and application.
