摘　　要：目的比较腹腔镜与开腹手术治疗伴有严重合并症结直肠癌患者的近期临床效果。方法回顾性分析福建医科大学附属第二医院2013年7月至2018年6月收治的228例术前伴有严重合并症的结直肠癌手术患者,依据手术方式分为腹腔镜组(173例)和开腹组(55例),比较分析2组患者围手术期情况。结果腹腔镜组术中出血量为(88.1±16.3)mL,术后并发症总发生率为9.83%,术后伤口感染发生率为1.73%,低于开腹组的(255.1±30.6)mL、21.82%、12.73%,差异均有统计学意义(t=52.273,P<0.001;χ^(2)=5.406,P=0.020;χ^(2)=12.026,P=0.002);术后肠功能恢复时间为(48.1±10.6)h,术后住院时间为(10.1±1.7)d,短于开腹组的(56.3±12.3)h、(12.5±2.3)d,差异均有统计学意义(t=4.803,P<0.001;t=8.331,P<0.001)。结论腹腔镜手术治疗伴有严重合并症患者安全有效,术后恢复快,值得临床推广。Objective To compare the short-term efficacy of laparoscopic and open surgery in the treatment of colorectal cancer patients with severe complications.Methods Retrospective analysis was performed on 228 cases of colorectal cancer patients with severe preoperative complications in the Second Affiliated Hospital of Fujian Medical University from Jul,2013 to Jun,2018,including 173 cases of the laparoscopic surgery group and 55 cases of the open surgery group,respectively.The perioperative conditions of the two groups were compared.Results In the laparoscopic surgery group,the intraoperative blood loss[(88.1±16.3)mL],the total postoperative complication incidence(9.83%),the postoperative wound infection incidence(1.73%)were lower than those in the open surgery group[(255.1±30.6)mL,21.82%,12.73%;t=52.273,P<0.001;χ^(2)=5.406,P=0.020;χ^(2)=12.026,P=0.002];the postoperative recovery time of intestinal function[(48.1±10.6)h]and the postoperative hospital stay[(10.1±1.7)days]were shorter than those of the open surgery group[(56.3±12.3)h,(12.5±2.3)d;t=4.803,P<0.001;t=8.331,P<0.001].Conclusion Laparoscopic surgery is safe and effect in the treatment of colorectal cancer patients with severe complications,which is worthy of clinical application.
