加速康复外科在后腹腔镜肾上腺肿瘤切除术中的应用效果分析

摘　　要：目的探讨加速康复外科运用于后腹腔镜肾上腺肿瘤切除术的安全性和有效性。方法选取2020年1—12月间该院收治的需行后腹腔镜肾上腺肿瘤切除术患者72例作为研究对象,按随机数字法分为参照组和研究组,各36例,参照组予以常规康复治疗,研究组予以加速康复外科治疗,比较两组间术后康复情况以及并发症的发生情况。结果研究组患者初次排气、初次进食和初次下床活动时间分别为(21.06±6.58)、(21.75±7.08)、(23.29±7.28)h,参照组患者初次排气、初次进食和初次下床活动时间(35.67±8.94)、(39.05±9.83)、(45.36±8.12)h,差异有统计学意义(t=7.897、8.568、12.142,P<0.05);研究组患者导尿管、引流管留置和住院时间分别为(19.32±8.09)h、(22.76±7.98)h、(3.64±0.76)d,适于参照组患者导尿管、引流管留置和住院时间分别为(57.41±9.33)h、(61.72±8.57)h、(5.89±1.12)d,差异有统计学意义(t=18.507、19.962、9.974,P<0.05)。研究组患者的并发症发生率为5.56%,明显低于参照组的27.78%,差异有统计学意义(χ^(2)=6.400,P<0.05)。结论对行后腹腔镜肾上腺肿瘤切除术患者予以加速康复外科有利于加速胃肠功能恢复,缩短引流管留置时间和住院时间,降低术后并发症发生概率,从而促进病情快速康复。Objective To investigate the safety and efficacy of accelerated rehabilitation surgery in retroperitoneal laparoscopic adrenalectomy.Methods From January to December 2020,72 patients in the hospital who needed retroperitoneal laparoscopic adrenal tumor resection were selected as the study object,they were divided into the reference group and the study group with 36 cases each according to the random number method.The reference group was given conventional rehabilitation treatment,and the study group was given accelerated rehabilitation surgery.The postoperative recovery,and occurrence of complications were compared between the two groups.Results The time of the patients in the study group for first exhaust,first eating and first getting out of bed were(21.06±6.58)h,(21.75±7.08)h,(23.29±7.28)h,earlier than the time of first exhaust,first eating and first getting out of bed in the reference group(35.67±8.94)h,(39.05±9.83)h,(45.36±8.12)h,the difference was statistically significant(t=7.897,8.568,12.142,P<0.05).The duration of catheterization,drainage catheter indwelling and hospital stay of the patients in the study group were(19.32±8.09)h,(22.76±7.98)h,and(3.64±0.76)d,shorter than the time of catheterization,drainage catheter indwelling and hospitalization of patients in the reference group(57.41±9.33)h,(61.72±8.57)h,and(5.89±1.12)d,the difference was statistically significant(t=18.507,19.962,9.974,P<0.05).The complication rate in the study group was 5.56%,which was significantly lower than the 27.78%in the reference group,the difference was statistically significant(χ^(2)=6.400,P<0.05).Conclusion Accelerated rehabilitation surgery for patients undergoing retroperitoneal laparoscopic adrenal tumor resection is beneficial to accelerate the recovery of gastrointestinal function,shorten the drainage tube indwelling time and hospital stay,reduce the probability of postoperative complications,and promote rapid recovery of the disease.
