摘  要:目的探讨rhBMP-2及兔颌外动脉血管束对壳聚糖(CS)/β-磷酸三钙(β-TCP)复合体修复兔下颌骨缺损的作用。方法于2018年6月—2019年3月选取36只左侧骨缺损的健康新西兰大白兔作为实验对象,按照随机数字表法将其分为3组,各12只,实验组在使用复合体修复的同时加入rhBMP-2并包埋血管束;对照A组使用复合体修复同时加入rhBMP-2;对照B组使用复合体修复同时并包埋血管束。在手术4周后、8周后及12周后每组分别处死3只兔子,并对尸体进行大体观察、X线检查、切片和骨密度观察,比较不同手术方案的效果。结果标本大体观察显示,复合材料在术后4周开始有骨膜附着,在术后8周开始有肌肉附着,术后12周骨膜和血管增生多。3组兔子的骨生长情况X线检查结果显示,第4周时3组比较,差异无统计学意义(P>0.05);第8周时,3组骨生长总分分别为(13.68±3.21)分、(7.16±1.01)分、(5.11±0.47)分,组间比较差异有统计学意义(P<0.05);第12周时,3组骨生长总分分别为(23.44±3.14)分、(13.32±2.12)分、(10.68±1.45)分,组间比较差异有统计学意义(P<0.05)。骨密度测量结果显示,实验组BMD术后8周为(0.25±0.03)g/cm^(2),12周为(0.32±0.03)g/cm^(2),均优于对照A组(0.22±0.02)、(0.29±0.02)g/cm^(2),B组(0.22±0.03)、(0.27±0.03)g/cm^(2),差异有统计学意义(t=2.882、2.882、2.449、4.082,P<0.05)。同时HE染色结果显示,实验组兔子的骨成熟各指标在术后4、8、12周时均高于对照A、B组,差异有统计学意义(P<0.05)。结论在使用CS/β-TCP对兔下颌骨缺损进行修复时加入rhBMP-2并包埋血管束可以促进骨头的生成。Objective To investigate the effect of rhBMP-2 and rabbit external mandibular artery vascular bundle on chitosan(CS)/β-tricalcium phosphate(β-TCP)composite repairing rabbit mandibular defects.Methods From June 2018 to March 2019,36 healthy New Zealand white rabbits with left bone defects were selected as experimental subjects,and they were divided into 3 groups with 12 rabbits in accordance with the random number table method.The experimental group was using complex repairs,at the same time,rhBMP-2 was added and the vascular bundle was embedded;the control group A used complex repair and rhBMP-2 was added at the same time;the control group B used the complex repair at the same time and embedded the vascular bundle.After 4 weeks,8 weeks,and 12 weeks after the operation,3 rabbits in each group were sacrificed,and the corpses were subjected to gross observation,X-ray examination,sectioning and bone density observation to compare the effects of different surgical procedures.Results The gross observation of the specimens showed that the composite material began to have periosteum attachment at 4 weeks after surgery,muscle attachment at 8 weeks after surgery,and more periosteum and blood vessel proliferation at 12 weeks after surgery.The results of X-ray examination of bone growth in the three groups showed that there was no statistically significant difference between the three groups at the 4th week(P>0.05);at the 8th week,the total scores of bone growth in the three groups were(13.68±3.21)points,(7.16±1.01)points and(5.11±0.47)points,respectively,the difference between groups was statistically significant(P<0.05);at the 12th week,the total scores of bone growth in the three groups were(23.44±3.14)points,(13.32±2.12)points and(10.68±1.45)points,respectively,the difference between groups was statistically significant(P<0.05).The results of bone mineral density measurement showed that the BMD of the experimental group was(0.25±0.03)g/cm^(2) at 8 weeks and(0.32±0.03)g/cm^(2) at 12 weeks,which were better than
