摘　　要：目的比较MR弥散加权成像(MR-DWI)与多层螺旋CT(MSCT)在急性缺血性脑卒中(AIS)诊断中的价值。方法110例AIS患者均进行MR-DWI、MSCT检查,比较两种检查方法的检出率及成像特征。结果MR-DWI的检出率显著高于MSCT(P<0.05)。缺血病灶区的平均表观扩散系数(ADC)值、脑血容量(CBV)及脑血流量(CBF)均显著低于组织正常区,达峰时间(TTP)显著长于组织正常区(P<0.05)。死亡组的平均ADC值、CBV及CBF均显著低于生存组,TTP显著长于生存组(P<0.05)。结论MR-DWI与MSCT均可准确评估AIS患者的病灶形成状态与疗效,但前者的检出率更高。Objective To compare the value of MR diffusion weighted imaging(MR-DWI)and multi-slice spiral CT(MSCT)in the diagnosis of acute ischemic stroke(AIS).Methods 110 AIS patients were examined by MR-DWI and MSCT.The detection rates and imaging characteristics of two examination methods were compared.Results The detection rate of MR-DWI was significantly higher than that of MSCT(P<0.05).The average apparent diffusion coefficient(ADC)value,cerebral blood volume(CBV)and cerebral blood flow(CBF)in the ischemic lesion area were significantly lower than those in the normal tissue area,and the time to peak(TTP)was significantly longer than that in the normal tissue area(P<0.05).The average ADC value,CBV and CBF in the death group were significantly lower than those in the survival group,and the TTP was significantly longer than that in the survival group(P<0.05).Conclusions Both MR-DWI and MSCT can accurately evaluate the lesion formation status and efficacy of AIS patients,but the former has higher detection rate.
