摘　　要：目的探讨一期经皮肝胆道镜碎石术(PTCSL)治疗肝胆管结石的效果及安全性。方法88例肝胆管结石患者随机分为两组各44例,观察组行一期PTCSL治疗,对照组行二期PTCSL治疗,比较两组的治疗效果及安全性。结果两组的结石清除率、手术时间及术中出血量比较,差异无统计学意义(P>0.05)。观察组的住院时间短于对照组(P<0.05)。观察组的术后胆道出血发生率为18.18%,高于对照组的4.55%(P<0.05)。结论一期经皮肝胆道镜碎石术治疗肝胆管结石的效果较好,可缩短术后住院时间,但应避免用于肝功能及凝血功能较差的患者。Objective To explore the effect and safety of one-stage percutaneous transhepatic cholangioscopic lithotripsy(PTCSL)for the treatment of hepatolithiasis.Methods 88 patients with hepatolithiasis were randomly divided into two groups,with 44 cases in each group.The observation group was treated with one-stage PTCSL,while the control group was treated with two-stage PTCSL.The curative effect and safety were compared between the two groups.Results No significant difference was found in stone clearance rate,operation time and intraoperative blood loss amount between the two groups(P>0.05).The hospitalization time of the observation group was shorter than that of the control group(P<0.05).The incidence of postoperative biliary bleeding of the observation group was 18.18%,higher than 4.55%of the control group(P<0.05).Conclusions One-stage PTCSL is effective for the treatment of hepatolithiasis,and can shorten the postoperative hospitalization time,but it should be avoided for patients with poor liver function and coagulation function.
