叙事医学的“生生”伦理指向及医患共同体建构

摘　　要：叙事医学是医学上一个新的“临床架构”,不仅朝向医学伦理本质而回归,更是直接命中了“生生”这一人类社会的最高伦理旨趣。这一旨趣从生命伦理学的角度为现代医患关系摆脱现有困窘并建构起正确的关系开阔了思路,将导向医患(生命)共同体的建构。基于“生生”意蕴,叙事医学视域下的医患共同体建构应当同时持有四个向度:作为利益共同体的“双赢”向度,作为情感共同体的“共情”向度,作为知识共同体的“同理”向度,作为伦理共同体的“共生”向度。Narrative medicine is a new"clinical framework"in medicine,which is not only a return to the ethical essence of medicine,but also directly hits the primary ethical purview of human society.From the perspective of bioethics,it disembarrasses the doctor-patient relationship and helps construct a correct one,namely the doctor-patient(life)community.Based on the meaning of"makes lives live",from the perspective of narrative medicine,the construction of doctor-patient life community should hold four dimensions:the"win-win"dimension as in interest community,the"empathy"dimension as in emotion community,the"shared cognition"dimension as in knowledge community,the"symbiosis"dimensions as in ethics community.
