TACE联合射频消融治疗原发性肝癌患者的临床效果

摘　　要：目的分析肝动脉化疗栓塞(TACE)联合射频消融(RFA)治疗原发性肝癌患者的临床效果。方法56例原发性肝癌患者,按照随机数字表法分为参照组和研究组,每组28例。参照组患者给予TACE治疗,研究组患者给予TACE联合RFA治疗。比较两组患者临床疗效、治疗前后的肝功能指标及甲胎蛋白(AFP)水平、生存率。结果研究组治疗总有效率85.71%高于参照组的60.71%,差异有统计学意义(P<0.05)。治疗后,研究组患者AFP(134.23±12.65)ng/ml、白蛋白(ALB)(31.59±3.57)g/L、总胆红素(TBIL)(51.27±3.67)μmol/L,均优于参照组的(269.48±19.63)ng/ml、(27.57±3.21)g/L、(60.73±3.54)μmol/L,差异均具有统计学意义(t=30.646、4.431、9.817,P=0.000、0.000、0.000<0.05)。研究组生存率高于参照组,差异有统计学意义(P<0.05)。结论临床中为原发性肝癌患者使用TACE联合RFA治疗后疗效较好,可以改善患者各项肝功能指标,并且能够有效提高患者的生存率,值得大力推广于临床试验中。Objective To analyze the clinical effect of transcatheter arterial chemoembolization(TACE)combined with radiofrequency ablation(RFA)in the treatment of primary liver cancer.Methods A total of 56 patients with primary liver cancer were divided into the reference group and the research group according to the random numerical table,with 28 cases in each group.Patients in the reference group were treated with TACE,and patients in the research group were treated with TACE and RFA.The clinical efficacy,liver function indexes and alpha-fetoprotein(AFP)level before and after treatment,and survival rate were compared between the two groups.Results The total effective rate of treatment 85.71%in the research group was higher than 60.71%in the reference group,and the difference was statistically significant(P<0.05).After treatment,patients in the research group had better AFP(134.23±12.65)ng/ml,albumin(ALB)(31.59±3.57)g/L,and total bilirubin(TBIL)(51.27±3.67)μmol/L than(269.48±19.63)ng/ml,(27.57±3.21)g/L,and(60.73±3.54)μmol/L in the reference group,and the differences were statistically significant(t=30.646,4.431,9.817;P=0.000,0.000,0.000<0.05).The survival rate of the research group was higher than that of the reference group,and the difference was statistically significant(P<0.05).Conclusion TACE combined with RFA is a good treatment for primary liver cancer patients.It can improve the liver function index of patients and effectively improve the survival rate of patients,which is worth popularizing in clinical trials.
