摘　　要：目的探讨乳腺癌原发灶及转移灶中雌激素受体(ER)、人表皮生长因子受体-2(Her-2)、细胞增殖指数(Ki-67)及孕激素受体(PR)表达差异。方法采用回顾性分析方法,选择2019年3月2020年3月河南科技大学第一附属医院病理科具有乳腺癌原发灶和前哨淋巴结转移灶的96例患者为研究对象,采用荧光原位杂交(FISH)检测和免疫组化法分析乳腺癌原发灶及前哨淋巴结转移灶的ER、PR、Her-2及Ki-67表达差异。结果Ki-67在乳腺癌原发灶的表达率比在前哨淋巴结转移灶高(P<0.05),表达一致率为70.83%,变化率为29.17%,表达变化对比无统计学差异(P>0.05);Her-2在前哨淋巴结转移灶和乳腺癌原发灶表达一致率为96.88%,变化率为3.12%,表达变化对比无统计学差异(P>0.05);ER在前哨淋巴结转移灶和乳腺癌原发灶表达一致率为100%,表达变化对比无统计学差异(P>0.05);PR在前哨淋巴结转移灶和乳腺癌原发灶表达一致率为95.83%,变化率为4.17%,表达变化对比无统计学差异(P>0.05)。结论乳腺癌原发灶与前哨淋巴结转移灶ER、Ki-67、PR及Her-2的表达情况一致性较好,但存在一定的异质性,可能与乳腺癌原发灶浸润前段分化较成熟的细胞等有关。Objective To investigate the estrogen receptor(ER),human epidermal growth factor receptor-2(Her-2),cell proliferation index(Ki-67)and progesterone receptor(PR)in primary breast cancer and metastases)Express differences.Methods Used retrospective analysis methods,96 patients with primary breast cancer and sentinel lymph node metastases in the Department of Pathology,the First Affiliated Hospital of Henan University of Science and Technology in March 2019 were selected as the research objects.Fluorescence in situ hybridization(FISH)Detection and immunohistochemical method to analyze whether the expressions of ER,PR,Her-2 and Ki-67 in the primary breast cancer and sentinel lymph node metastases are different.Results The expression rate of Ki-67 in breast cancer primary tumors was higher than that in sentinel lymph node metastases(P<0.05),the expression agreement rate was 70.83%,the change rate was 29.17%,and the expression changes were not significantly different(P>0.05);The agreement rate of Her-2 expression in sentinel lymph node metastasis and breast cancer primary tumor was 96.88%,and the change rate was 3.12%.There was no significant difference in expression changes(P>0.05);ER was in sentinel lymph node metastasis and breast The expression consistency rate of primary cancer was 100%,and there was no significant difference in expression changes(P>0.05);the expression consistency rate of PR sentinel lymph node metastasis and breast cancer primary tumor was 95.83%,the change rate was 4.17%,and the expression change There was no significant difference in comparison(P>0.05).Conclusion The expressions of ER,Ki-67,PR and Her-2 in primary breast cancer and sentinel lymph node metastases are in good agreement,but there is a certain heterogeneity,which may be more maturely differentiated from the anterior segment of the primary breast cancer infiltration Cells and so on.
