摘　　要：目的探讨卵巢甲状腺肿类癌的临床病理特征。方法回顾性分析2010年9月至2020年9月福建省妇幼保健院收治的5例卵巢甲状腺肿类癌病例的诊疗过程、病理特征和免疫表型,并复习文献。结果5例患者年龄16~57岁,以盆腔包块、腹痛及阴道不规则出血症状为主,均行手术治疗,切除的卵巢肿物均增大,呈囊实性,直径约6.0~16.0 cm。免疫组化显示:突触素(Syn)、嗜铬蛋白(CgA)均呈现不同程度阳性表达,甲状腺球蛋白(Tg)有4例阳性,1例阴性。术后病理提示卵巢甲状腺肿类癌2例,类癌合并成熟性畸胎瘤3例。结论卵巢甲状腺肿类癌十分罕见,患者多无典型症状,且类癌成分多与甲状腺组织混杂于畸胎瘤中,容易漏诊,具有特征性的病理和免疫组织化学特点,预后较好。Objective To explore the clinicopathological characteristics of strumal carcinoid of the ovary.Methods Five cases strumal carcinoid of the ovary in Fujian Maternity and Child Health Hospital from September 2010 to September 2020 were selected,and the diagnosis and treatment process,pathological characteristics and Immunophenotype were analyzed retrospectively.The literatures were reviewed.Results The 5 patients were 16 to 57 years old.The main symptoms were pelvic mass,abdominal pain and irregular vaginal bleeding.All of them underwent surgical treatment,the removed ovarian masses were all enlarged and cystic,with a diameter of 6.0-16.0 cm.Immunohistochemistry showed that synaptophysin(Syn)and and chromogranin A(CgA)all showed positive expression to varying degrees.Thyroglobulin(Tg)was positive in 4 cases and 1 case was negative.Postoperative pathology directly revealed 2 cases of strumal carcinoid of the ovary and 3 cases of carcinoid complicated with mature teratoma.Conclusion Strumal carcinoid of the ovary is very rare,most patients have no typical symptoms,and carcinoid components are mostly mixed with thyroid tissue in teratoma,which is easy to be missed,with characteristic pathological and immunohistochemical characteristics,and a good prognosis.
