摘　　要：目的探讨胸腔外异位型支气管源性囊肿(BC)分布特点及影像学表现。方法回顾性分析经术后病理证实的14例胸腔外(腹部、颈部)支气管源性囊肿的术前CT和临床资料,8例行增强CT扫描(7例行CT平扫加增强,1例直接增强),6例仅行CT平扫,分析病灶的分布、形态及CT表现。结果14例患者中,发生于腹部7例(左侧肾上腺区2例,贲门2例,肝胃间隙1例,胰腺体部上方1例,右侧肾后间隙1例);颈部7例(甲状腺左侧叶前方2例,左侧胸锁乳突肌内、外侧各1例,右颈阔肌内侧1例,右侧颌下腺后方1例,颈前甲状舌骨区1例);所有病例均为单发且边界清楚;12例表现为类圆形,1例为卵圆形,1例为梭形;4例囊壁可见斑点、结节状钙化;病灶平扫平均CT值大于20 HU者8例,平均CT值小于等于20 HU者5例,增强扫描中仅2例表现为囊壁轻度强化。结论胸腔外异位型BC多分布在中线近气管支气管树的区域,其CT表现具有一定的特征性。Objective To investigate the distribution and CT features of extrathoracic ectopic bronchogenic cysts.Methods The clinical record and preoperative unenhanced(13)and enhanced CT(8)of 14 patients with pathologically confirmed abdominal(7)and cervical(7)bronchogenic cysts were analyzed retrospectively.The distribution,morphology and CT features of the lesions were reviewed.Results The 7 abdominal lesions included left adrenal gland(2),gastric cardia(2),hepatogastric space(1),above the body of pancreas(1),and right posterior renal space(1).The 7 cervical lesions were in the anterior left lobe of the thyroid(2),left medial(1)and lateral(1)sternocleidomastoid muscles,inner side of the right platysma muscle(1),right posterior submandibular gland(1),and anterior thyrohyoid region(1).All lesions were solitary with well-defined boundary,round(12),oval(1)or spindle(1)in shape on CT with speckled or nodular calcifications on the cyst wall(4).The average unenhanced CT value was>20 HU in 8 and≤20 HU in 5 with mild enhancement of the cyst wall in 2/8 patients.Conclusion Extrathoracic ectopic bronchogenic cyst is mostly distributed in the midline near the tracheobronchial tree with characteristic CT features.
