刺血拔罐法联合白头翁汤加味口服合并灌肠治疗急性放射性肠炎临床观察

摘　　要：目的观察刺血拔罐法联合白头翁汤加味口服合并灌肠治疗急性放射性直肠炎临床疗效。方法选择90例急性放射性直肠炎患者按照随机数字表法分为3组:西医外治组(给予西药灌肠)、中医外治组(给予中药灌肠+刺血拔罐)、中医内服外治组(给予中药灌肠+刺血拔罐+中药口服)。比较3组临床疗效,治疗前后临床证候积分及药物不良反应发生率。结果中医内服外治组中医临床疗效、治疗后临床证候积分、血红蛋白、白细胞、血小板及KPS评分均显著优于西医外治组(P<0.05);在中医临床疗效,治疗后脓血便、肛门灼热、白细胞、血小板及卡氏功能状态评分标准(KPS)上优于中医外治组(P<0.05);3组药物不良反应发生率比较差异无统计学意义(P>0.05)。结论刺血拔罐法联合白头翁汤加味口服合并灌肠治疗急性放射性直肠炎能够控制病情进展,缓解临床症状,提高患者生活质量且安全性值得认可。Objective:To observe the clinical efficacy of pricking blood and cupping combined with modified Baitouweng Decoction combined with enema in the treatment of acute radiation proctitis.Methods:A total of 48 patients with acute radiation proctitis were randomly divided into three groups:western medicine external treatment group(western medicine enema),traditional Chinese medicine external treatment group(traditional Chinese medicine enema+pricking blood cupping),traditional Chinese medicine internal and external treatment group(traditional Chinese medicine enema+pricking blood cupping+oral Chinese Medicine).The clinical efficacy,clinical syndrome score and incidence of adverse drug reactions were compared among the three groups.Results:Pricking blood cupping+modified Baitouweng Decoction+enema was significantly better than western medicine external treatment group in TCM clinical efficacy,clinical syndrome score,hemoglobin,leukocyte,platelet and KPS score after treatment(P<0.05),and the TCM clinical efficacy,purulent bloody stool,anal burning,white blood cells,platelets and KPS scores after treatment were better than those in TCM external treatment group(P<0.05);there was no significant difference in the incidence of adverse drug reactions among the three groups(P>0.05).Conclusion:Pricking blood cupping+modified Baitouweng Decoction+enema in the treatment of acute radiation proctitis can control the progress of the disease,relieve the clinical symptoms,improve the quality of life of patients,and is worthy of recognition.
