中医肿意膏外敷治疗周围性面瘫的临床观察

摘　　要：目的探索肿意膏外敷治疗周围性面瘫疗效。方法选取82名周围性面瘫伴耳部疼痛患者,观察组采用常规针刺法结合肿意膏外敷法贴于患者患侧耳部周围,对照组则用常规针刺耳周穴位,每日1次,治疗14 d后观察患者的治疗效果及其对相关指标的影响。结果治疗后两组面瘫局部外周血IgE均较治疗前明显升高(P<0.05),且观察组升高较对照组明显(P<0.05)。两组患者局部外周血MMP-9及SP水平均较治疗前明显降低(P<0.05),且观察组降低较对照组明显(P<0.05)。两组Portmann评分及FDIP评分均较治疗前明显增高(P<0.05),且观察组高于对照组(P<0.05)。观察组平均耳部疼痛开始缓解时间及疼痛持续时间均明显短于对照组(P<0.05)。观察组治疗10 d后平均疼痛缓解程度明显高于对照组(P<0.05)。观察组平均疼痛症状消失所需时间明显少于对照组(P<0.05);观察组临床总有效率为95.12%,明显高于对照组的85.37%(P<0.05)。结论肿意膏运用于周围性面瘫急性期,能解决面瘫症状及所伴的耳痛,缩短病程,临床疗效显著,提高了治愈率。Objective:To explore the effect of external application of Zhongyi Ointment on peripheral facial paralysis and related indexes of patients.Methods:A total of 82 patients with peripheral facial paralysis with ear pain were divided into 2 groups.The observation group was treated with conventional acupuncture combined with external application of Zhongyi Ointment,while the control group was treated with routine pricking points around the ear,once a day.After 14 days of treatment,the therapeutic effect of patients with facial paralysis and its influence on related indicators were observed.Results:After treatment,the local peripheral blood IgE of facial paralysis in the two groups was significantly higher than that before treatment(P<0.05),and the increase in the observation group was significantly higher than that in the control group(P<0.05);the local peripheral blood MMP-9 and SP concentrations of the two groups were both significantly lower than before treatment(P<0.05),and the observation group was significantly lower than the control group(P<0.05);the Portmann score and FDIP score of the two groups were significantly higher than before treatment(P<0.05),and the observation group was significantly higher than the control group(P<0.05);the average ear pain relief time and pain duration of the observation group were significantly shorter than those of the control group(P<0.05);the average pain relief of the observation group after 10 days of treatment was significantly higher than that of the control group(P<0.05);the average time required for the disappearance of pain symptoms in the observation group was significantly less than that in the control group(P<0.05);the total effective rate in the observation group was 95.12%,which was significantly higher than 85.37%of the control group(P<0.05).Conclusion:Zhongyi Ointment applied in the acute stage of peripheral facial paralysis can solve the symptoms of facial paralysis and the accompanying earache,shorten the course of the disease,have significant clinical effects,
