摘  要:目的应用数字化的诊疗设计和3D打印制作无托槽隐形矫治器从而解决简单牙列不齐等问题。评价基于数字光处理技术3D打印牙列模型的精确性,论证该数字化正畸流程可行性。方法通过选取轻度间隙不调、扭转等不同类型的简单不齐病例20例,3shape Trios全称“口内扫描”获得牙列信息后应用3shape Ortho Analyzer软件进行排牙设计并导出各阶段步骤模型数据用于3D打印和压制无托槽矫治器;在85个上下领牙列数字化设计模型和对应3D打印模型上选取对应左右中切牙、尖牙和磨牙的牙冠高度、牙冠宽度尖牙和磨牙间牙弓宽度、牙弓长度分别进行软件及手工测量。采用组内相关系数(ICC)对重复测量信度进行评估;采用配对t检验分析两种模型在各测量指标上有无差异以检验模型精确性。结果软件测量数字化设计模型和手工测量3D打印实体模型结果的ICC均>0.95,测量者应用软件及手工测量的可重复性均较高。两种模型在除外牙冠高度的其他各测量指标上的一致性高,配对t检验结果P>0.05,整体偏差范围在(-0.01±0.12)mm至(0.11±0.31)mm之间。结论基于数字光处理技术以10μm层厚精度3D打印的牙列模型精确性较高,可用于临床简单病例隐形矫治器制作。Objective The objective of this study was to assess the accuracy and feasibility of three dimensional(3D)printing dental models based on Digital Light Processing(DLP)used for the fabrication of clear aligners in mild malocclusions.Methods T wenty patients with different types of simple irregularities were selected and intra-oral scanned for the digital dental models with 3shape Trios.The 3shape Ortho Analyzer software was used to setup the digital models and the tooth movement staging.The step model data were derived for 3D printing and aligner fabrication.Eighty-five upper and lower jaw dentition digital models and 3D-printed models were compared digitally or manually with the selected corresponding crown height,crown width,arch width and length measurements to test the consistency and acuracy of the 3D-printed model.The intraclass correlation coefficients(ICC)was used to evaluate the reliability of the two measurement methods.Paired t tests were used to detect significant differences in the measurements between the 3D printed models and the original digital models.Results The intraclass correlation coefficients for all the comparisons made between the 3D replicas and the original digital models were greater than 0.95.This indicated an excellent agreement.No statistically significant differences could be detected between the 3D-printed models and their corresponding digital models except for crown height measurements.The overall difference was ranging from(-0.01±0.12)mm to(0.11±0.31)mm.Conclusions The accuracy of the 3D models produced by the DLP techniques at 10μm layer heights for the fabrication of clear aligners in mild malocclusions was clinically acceptable.
