急性白血病气阴两虚夹瘀证舌象特点及血常规指标变化关系研究

摘　　要：目的:观察急性白血病气阴两虚夹瘀证患者治疗前后舌象及血常规指标的变化,为有效防治提供参考。方法:选择急性白血病48例气阴两虚夹瘀证患者为研究对象,观察其治疗前后的舌质、舌苔及舌形,检测治疗前后的血常规指标。结果:研究对象治疗前舌质深红,舌苔多黄厚腻,舌形多为齿痕舌;治疗后舌质变淡,舌苔变薄白,齿痕和裂纹舌减少,血细胞、红细胞、血小板等血细胞数目和中性细胞、淋巴细胞和单核细胞等血细胞分类比率较正常值升高或降低,治疗后上述6项指标异常率显著减少,差异有统计学意义(P<0.05)。舌象变化与血常规指标变化规律基本一致。结论:急性白血病气阴两虚夹瘀证患者的舌象存在明显的血瘀征象特点,急性白血病患者白细胞数目、淋巴细胞和单核细胞比率显著增高,血小板和红细胞数目及中性细胞比率显著减少;活血化瘀和益气养阴的辨证治疗消除部分瘤体,使血瘀舌象和血液流变学得到显著改善,全面观察舌象,并与血液流变学检测共诊合参,可获得更多临床信息,对病情判断、疗效评估有一定指导意义。Objective:To observe the changes of tongue and blood routine indexes in patients with Qiyin deficiency and blood stasis syndrome of acute leukemia before and after treatment,so as to provide reference for effective prevention and treatment.Methods:Among the acute leukemia patients 48 patients with Qiyin deficiency and blood stasis syndrome were selected as the research object,and the tongue quality,tongue coating and tongue before and after treatment were observed.Shape,testing blood routine indicators before and after treatment.Results:before treatment,the subjects’tongue was crimson,the tongue coating was yellow,thick and greasy,and the shape of the tongue was mostly tooth marks.After treatment,the tongue became pale,the tongue coating became thin and white,the tooth marks and cracked tongue decreased,the number of blood cells such as blood cells,red blood cells and platelets and the classification ratio of blood cells such as neutrophils,lymphocytes and monocytes increased or decreased compared with the normal value.After treatment,the abnormal rate of the above six indexes decreased significantly(P<0.05).The changes of tongue image were basically consistent with the changes of blood routine indexes.Conclusion:Patients with acute leukemia with Qiyin deficiency and blood stasis syndrome have obvious signs of blood stasis on their tongue.The number of white blood cells,lymphocytes and monocytes in patients with acute leukemia are significantly increased,and the number of platelets and red blood cells and the ratio of neutrophils are significantly reduced.The dialectical treatment of activating blood to remove blood stasis and replenishing qi and nourishing yin eliminates part of the tumors,and significantly improves the blood stasis and tongue picture and hemorheology.Comprehensive observation of the tongue picture and co-diagnosis with hemorheology test can be obtained more clinical information has certain guiding significance for disease judgment and efficacy evaluation.
