摘　　要：目的:探究Er:YAG激光治疗牙周炎的疗效及对牙周指标、炎性因子的改善效果。方法:选择2017年6月-2020年6月在南京市口腔医院就诊的120例慢性牙周炎患者的病例资料进行回顾性分析。所有患者根据所接受治疗方法的不同分为对照组和观察组,每组60例。对照组采用常规基础治疗,观察组在对照组治疗方法基础上使用Er:YAG激光进行治疗(去除牙龈下结石、牙周袋内杀菌)。对比两组的治疗效果、牙周指标[牙龈指数(GI)、牙菌斑指数(PLI)、牙槽骨吸收程度、牙周袋深度(PD)]、龈沟液炎性因子指标[白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、可溶性细胞间黏附分子-1(sICAM-1)]、血清氧化应激指标[丙二醛(MDA)、超氧化物歧化酶(SOD)]及不良反应情况。结果:观察组治疗有效率100.00%,高于对照组的90.00%,两组比较差异有统计学意义(P<0.05)。治疗后3个月,观察组的GI、PLI、牙槽骨吸收程度、PD、龈沟液的IL-6、IL-8、sICAM-1、MDA均低于对照组,观察组的SOD、美学效果指标评分均高于对照组,差异有统计学意义(P<0.05)。两组治疗过程中的不良反应发生率比较,差异无统计学意义(P>0.05)。结论:采用Er:YAG激光联合常规基础方法治疗慢性牙周炎患者,可改善治疗效果及各类牙周指标,降低局部的炎性反应和氧化应激反应,具有良好的安全性。Objective To explore the effect of Er:YAG laser on periodontal indicators,inflammatory factors of periodontitis.Methods A retrospective analysis of the data of 120 patients with chronic periodontitis who visited Nanjing Stomatological Hospital from June 2017 to June 2020.All patients were divided into the control group and the observation group according to the differences in the treatment methods they received,with 60 cases in the control group and 60 cases in the observation group.The control group was treated with conventional basic treatment,and the observation group was treated with Er:YAG laser based on the treatment method of the control group(removal of subgingival stones and sterilization in the periodontal pocket).Compare the treatment effects of the two groups,periodontal indicators[gingival index(GI),dental plaque index(PLI),degree of alveolar bone resorption,periodontal pocket depth(PD)],gingival crevicularfluid inflammatory factor indicators[leukocytosis IL-6(IL-6),interleukin-8(IL-8),soluble intercellular adhesion molecule-1(sICAM-1)],serum oxidative stress indicators[malondialdehyde(MDA),superoxide Dismutase(SOD)],adverse reactions.Results The effective rate of treatment in the observation group was 100.00%,which was higher than 90.00%in the control group(P<0.05).Three months after treatment,the observation group's GI,PLI,alveolar bone absorption,PD,and gingival crevicularfluid IL-6,IL-8,sICAM-1,MDA,were all lower than those of the control group.The observation group's SOD,The aesthetic effect index scores were higher than those of the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups during treatment(P>0.05).Conclusion In the treatment of patients with chronic periodontitis,the use of Er:YAG laser combined with conventional basic methods to complete the treatment can improve the treatment effect and various periodontal indicators,and reduce local inflammatory reactions and oxidative stress reactions,has good safety
