钡餐造影及CT扫描在食管癌患者术前分期评估中的临床价值

摘　　要：目的探讨钡餐造影及电子计算机断层扫描(CT)在食管癌患者术前分期评估中的临床价值。方法选取2019年6月至2020年6月民权县人民医院收治的65例食管癌患者,均行X线钡餐造影及CT检查。比较两种方法诊断食管癌分期检出率及二者联合检查食管癌的分期准确度,采用Kappa分析二者单独及联合检查与病理检查结果的一致性。结果X线钡餐造影对早期食管癌的检出率明显高于CT检查(P<0.05),中晚期食管癌检出率明显低于CT检查(P<0.05);X线钡餐造影分期诊断结果与病理检查结果不具有一致性(Kappa=0.048),CT检查与病理检查结果一致性差(Kappa=0.100),二者联合检查与病理检查结果一致性较好(Kappa=0.738);X线钡餐造影、CT检查及二者联合检查诊断食管癌Ⅰ~Ⅱ期准确度比较差异无统计学意义(P>0.05),而二者联合检查诊断食管癌Ⅲ~Ⅳ期准确度明显高于单独检查(P<0.05)。结论X线钡餐造影在早期食管癌诊断价值较高,CT检查在中晚期食管癌诊断中价值较高,二者联合检查可提高食管癌Ⅲ~Ⅳ期诊断准确度,对临床食管癌的分期诊断有较高的参考价值。Objective To explore the clinical value of barium radiography and CT in preoperative staging evaluation of patients with esophageal cancer.Methods 65 patients with esophageal cancer treated in Minquan County People’s Hospital from June 2019 to June 2020 were selected and examined by barium radiography and CT.The detection rates of esophageal cancer staging of the two methods,and staging accuracy of the single and joint examination of two methods were compared,and Kappa was used to analyze the consistency between the results of single,joint examinations and their pathological examination.Results The detection rate of early esophageal cancer of barium radiography was obviously higher than that of CT(P<0.05);and the detection rate of middle and late esophageal cancer of barium radiography was obviously lower than that of CT(P<0.05).The results of barium radiography staging were not consistent with those of pathological examination(Kappa=0.048);the consistency between CT and pathological examination was poor(Kappa=0.100);the results of joint examination and pathological examination were consistent(Kappa=0.738).There was no significant difference in the accuracy of barium radiography,CT,and their joint examination in the diagnosis of stageⅠ~Ⅱesophageal cancer(P>0.05);and the diagnosis accuracy of stageⅢ~Ⅳof joint examination was obviously higher than that of single diagnosis(P<0.05).Conclusion Barium radiography is of high value in the diagnosis of early esophageal cancer;CT is of high value in the diagnosis of advanced esophageal cancer;the joint examination can effectively improve the accuracy of stageⅢ~Ⅳof esophageal cancer,and has high reference value for the staging diagnosis of clinical esophageal cancer.
