癫痫类型及抗癫痫药物对病人大脑皮质兴奋性影响

摘　　要：目的探讨癫痫类型及抗癫痫药物(AEDs)对癫痫病人大脑皮质兴奋性的影响。方法选取局灶性癫痫病人31例和全面性癫痫病人21例作为研究对象,根据其服药情况分为局灶未服药组(n=14)、局灶服药组(n=17)、全面未服药组(n=13)、全面服药组(n=8),以健康体检者15例作为正常对照组。应用经颅磁刺激技术检测各组静息运动阈值、运动诱发电位波幅及潜伏期并进行比较。结果局灶未服药组与局灶服药组、局灶未服药组与全面服药组间静息运动阈值比较差异有统计学意义(t=-2.059、-2.059,P<0.05);局灶服药组及局灶未服药组运动诱发电位波幅水平与正常对照组相比均明显升高(t=2.375、2.641,P<0.05)。结论局灶性癫痫病人大脑皮质兴奋性较正常人群明显升高,且AEDs对局灶性癫痫病人大脑皮质兴奋性的抑制作用更加明显。Objective To investigate the effect of different types of epilepsy and anti-epileptic drugs(AEDs)on cerebral cortical excitability.Methods A total of 31 patients with focal epilepsy and 21 patients with generalized epilepsy were enrolled as subjects,and according to their medication,they were divided into focal epilepsy without medication group with 14 patients,focal epilepsy with medication group with 17 patients,generalized epilepsy without medication group with 13 patients,and generalized epilepsy with medication group with 8 patients.A total of 15 individuals who underwent physical examination were enrolled as normal control group.Transcranial magnetic stimulation was used to measure resting motor threshold(RMT)and amplitude and latency of motor evoked potential(MEP),which were then compared between groups.Results There was a significant difference in RMT between the focal epilepsy without medication group and the focal epilepsy with medication group(t=-2.059,P<0.05)and between the focal epilepsy without medication group and the generalized epilepsy with medication group(t=-2.059,P<0.05),and compared with the normal control group,the focal epilepsy with medication group and the focal epilepsy without medication group had a significantly higher amplitude of MEP(t=2.375,2.641;P<0.05).Conclusion The patients with focal epilepsy have significantly higher cerebral cortical excitability than the normal population,and AEDs can significantly inhibit cerebral cortical excitability in patients with focal epilepsy.
