摘　　要：目的探讨改良一片式可折叠后房型人工晶体睫状体平坦部缝线固定术的临床疗效。方法采用回顾性病例对照研究,对2011年3月—2016年4月在青岛市市立医院眼科中心行人工晶体缝线固定术的56例病人资料进行分析,依据人工晶体缝线固定部位的不同分成睫状体平坦部(角巩膜缘后3.0 mm)固定组和睫状沟(角巩膜缘后1.5 mm)固定组。比较两组病人的基线资料;行裂隙灯、前房角镜、超声生物显微镜(UBM)检查,比较两组术后色素播散综合征(PDS)、色素播散性青光眼(PG)、虹膜夹持(IC)和反向瞳孔阻滞(RPB)等并发症的发生情况。结果共有56例59只眼被纳入研究,其中睫状体平坦部固定组35例36只眼,睫状沟固定组21例23只眼,两组病人的基线资料比较差异无显著性(P>0.05)。睫状沟固定组术后并发症PDS、PG、RPB、IC等的发生率高于睫状体平坦部固定组,差异均有统计学意义(χ^(2)=3.864~43.382,P<0.01)。两组术后其他并发症比较差异无显著性(P>0.05)。结论一片式可折叠后房型人工晶体睫状体平坦部缝线固定术术后并发症较少,是一种安全可靠的手术方式。Objective To investigate the clinical effect of pars plana suture fixation for single-piece acrylic(SPA)foldable posterior chamber intraocular lenses(PC-IOLs).Methods A retrospective case-control study was performed to analyze the cli-nical data of 56 patients who underwent suture fixation of intraocular lens in Department of Ophthalmology,Qingdao Municipal Hospital,from March 2011 to April 2016.According to the position for suture fixation of intraocular lens,the patients were divi-ded into pars plana fixation group(at 3.0 mm posterior to the limbus)and ciliary sulcus fixation group(at 1.5 mm posterior to the limbus).Baseline data were compared between the two groups.Slit lamp examination,gonioscopy,and ultrasound biomicroscopy(UBM)were performed,and the two groups were compared in terms of the incidence rates of postoperative complications such as pigment dispersion syndrome(PDS),pigmentary glaucoma(PG),iris capture(IC),and reverse pupillary block(RPB).ResultsA total of 56 patients(59 eyes)were enrolled in this study,with 35 patients(36 eyes)in the pars plana fixation group and 21 patients(23 eyes)in the ciliary sulcus fixation group,and there were no significant differences in baseline data between the two groups(P>0.05).Compared with the pars plana fixation group,the ciliary sulcus fixation group had significantly higher incidence rates of the postoperative complications PDS,PG,RPB,and IC(χ^(2)=3.864-43.382,P<0.01),while there were no significant differences in other postoperative complications between the two groups(P>0.05).Conclusion Pars plana suture fixation for SPA foldable posterior chamber intraocular lenses has few postoperative complications and is thus a safe and reliable surgical procedure.
