二陈汤加味联合肠内营养支持对中晚期胃肠道恶性肿瘤患者化疗期间营养状态及生活质量的影响

摘　　要：目的观察二陈汤加味联合肠内营养支持对中晚期胃肠道恶性肿瘤患者化疗期间营养状态及生活质量的影响。方法将122例中晚期胃肠道恶性肿瘤患者按照随机数字表法分为2组,均予标准化疗方案。对照组61例予肠内营养支持,治疗组61例在对照组治疗基础上联合二陈汤加味治疗,2组均治疗6周。观察2组治疗前后营养指标、血红蛋白(Hb)、白细胞计数(WBC)、血小板计数(PLT),肝功能指标血清白蛋白(Alb)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)及肾功能指标血清肌酐(Cr)、尿素氮(BUN)变化,观察2组化疗不良反应。结果2组体质量指数(BMI)、营养不良评分、生活质量评分差值比较差异均有统计学意义(P<0.05),治疗组营养指标优于对照组。2组Hb、Alb差值比较差异均有统计学意义(P<0.05),WBC、PLT差值比较差异均无统计学意义(P>0.05)。2组ALT、AST、Cr及BUN差值比较差异均无统计学意义(P>0.05)。2组发热、贫血、胃肠道反应、乏力发生率比较差异有统计学意义(P<0.05),治疗组低于对照组。结论二陈汤加味联合肠内营养支持治疗中晚期胃肠道恶性肿瘤,能提高患者化疗期间营养状态及生活质量。Objective To investigate the effect of modified Erchen Decoction combined with enteral nutrition support on nutritional status and quality-of-life of patients with advanced gastric cancer(AGC)during chemotherapy.Methods Totally 122 AGC patients during chemotherapy were equably divided into treatment group and control group as random number table method.On basis of enteral nutrition support,the treatment group was treated with modified Erchen Decoction.By 6-week treatment,the key observition was nutritional indicators,hemoglobin(Hb),white blood cell count(WBC),platelet count(PLT)and albumin(Alb);and liver function indicators,including alanine aminotransferase(ALT),aspartate aminotransferase(AST);as well as renal function indicators,involving creatinine(Cr)and urea nitrogen(BUN),adverse reactions of chemotherapy between groups before and after treatment.Results The difference was statistically significant in the body mass index(BMI),malnutrition score and quality-of-life score between groups(P<0.05),nutritional indicators were better in treatment group than in control group;there was significant difference being found in the comparison of Hb and Alb between groups(P<0.05),and the difference between WBC and PLT was not statistically significant(P>0.05);the difference was not statistically significant in the ALT,AST,Cr and BUN between groups(P>0.05),the difference between fever,anemia,gastrointestinal reactions,fatigue was statistically significant(P<0.05);adverse reactions of chemotherapy was decreased in treatment group when compared with cortrol group.Conclusion For AGC patients,mdified Erchen Dcoction combined with nutritional support is more effective,which is helpful to improve nutritional status and quality-of-life of patients during chemotherapy.
