小儿舌系带矫正术术后麻醉苏醒时间的影响因素分析

摘　　要：目的:分析影响小儿舌系带矫正术术后麻醉苏醒时间的危险因素。方法:回顾性分析2015年1月~2019年12月在我科行舌系带矫正术的813例患儿病例。收集患儿的人口学特征、术中麻醉药物使用情况、术中气道管理方式及手术时间、苏醒时间和复苏室停留时间等资料。对患儿术后麻醉苏醒时间和复苏室停留时间的影响因素进行分析。结果:术后苏醒时间延长的危险因素包括丙泊酚、咪达唑仑的使用,舒芬太尼用量增加以及术中气道管理方式。术后复苏室停留时间延长的危险因素包括丙泊酚、右美托咪定的使用,舒芬太尼用量增加以及术中气道管理方式。结论:丙泊酚、咪达唑仑、右美托咪定的使用以及舒芬太尼的用量、术中气道管理方式是影响小儿舌系带矫正术术后苏醒时间的危险因素。但由于本研究未评价苏醒期躁动等苏醒质量相关指标,因此上述药物的使用还需下一步前瞻性研究进行综合评价。Objective:To determine the factors that influence the awakening time from anesthesia in children undergoing frenuloplasty.Methods:A total of 813 children that underwent frenuloplasty between January 2015 to December 2019 were included.The following information was collected:demographic characteristics,use of anesthetics,airway management,operation time,time to awakening and length of stay in recovery room.The factors that influence the time to awakening and length of stay in recovery room were analyzed.Results:Longer time to awakening was associated with the use of propofol,midazolam,increased sufentanil dosage and airway management.Longer duration of stay in recovery room was associated with the use of propofol,dexmedetomidine,increased sufentanil dosage,and airway management.Conclusion:Use of propofol,midazolam,dexmedetomidine,increased sufentanil dosage and airway management were associated with prolonged anesthesia recovery in children undergoing frenuloplasty.The present study did not evaluate the effects of these drugs on emergence agitation,therefore,our findings need to be interpreted with caution.
