摘 要:目的探讨膳食纤维补充剂对2型糖尿病(T2DM)患者的疗效。方法选择2020年10月至2021年4月在北京大学深圳医院内分泌科门诊就诊的60例T2DM患者, 按照抛硬币法随机分为对照组(36例)和试验组(24例)。对照组维持原糖尿病饮食及生活习惯, 试验组在原饮食习惯基础上第4至10天随餐服用膳食纤维粉共18.9 g/d。干预前予安装持续葡萄糖监测仪器, 计算平均血糖值(MBG)、血糖的曲线下面积(GAUC)、葡萄糖在目标范围内时间(TIR)百分比、葡萄糖高于目标范围内时间(TAR)百分比、葡萄糖低于目标范围内时间(TBR)百分比、血糖标准差(SDBG)、平均血糖波动幅度(MAGE)。采用t检验或Wilcoxon符号秩检验比较两组受试者干预前后各指标的变化。结果膳食纤维补充剂干预后, 与对照组比较, 试验组的MBG、TAR、GAUC降低, 差异均具有统计学意义(P<0.05), 而两组间的TIR、TBR差异均无统计学意义(P>0.05)。与对照组比较, 试验组的SDBG、MAGE降低, 差异均有统计学意义(P<0.01)。试验组中与干预前比较, 干预后的MBG、TAR、TBR、GAUC下降, TIR升高, 差异均有统计学意义(P<0.05);干预后的SDBG、MAGE较干预前有所下降, 差异均有统计学意义(P<0.01)。结论补充膳食纤维能改善T2DM患者整体平均血糖, 改善日内血糖的波动, 对血糖管理具有良好的调节作用。Objective To investigate the therapeutic effect of dietary fiber supplement on type 2 diabetes mellitus(T2DM).Methods A total of 60 T2DM patients admitted to the Department of Endocrinology,Peking University Shenzhen Hospital from October 2020 to April 2021 were randomly divided into the control group(36 cases)and the experimental group(24 cases)according to the coin flipping.The control group maintained the original diabetic diet and lifestyle,while the experimental group took dietary fiber powder 18.9 g/d with meals on the basis of the original diet.All subjects were installed with continuous glucose monitoring instrument before intervention,and we calculated mean blood glucose(MBG),area under the curve of blood glucose(GAUC),time in range(TIR),time above target glucose range(TAR),time below target glucose range(TBR),standard deviation of blood glucose(SDBG),and mean amplitude of glycemic excursion(MAGE).The changes of the indexes before and after intervention were compared.Results After dietary supplement intervention,compared with the control group,MBG,TAR and GAUC were decreased in the experimental group,and the difference between the two groups was statistically significant(P<0.05),while TIR and TBR showed no significant difference between the two groups(P>0.05).Compared with the control group,the SDBG and MAGE of the experimental group were decreased,which was statistically significant(P<0.01).The MBG,TAR,TBR and GAUC of the experimental group decreased after intervention,while TIR increased,with statistical significance(P<0.05).SDBG and MAGE were lower after intervention,and the difference was statistically significant(P<0.01).Conclusions Dietary fiber supplements could improve the overall average blood glucose of type 2 diabetic patients,ameliorate the intra-day blood glucose fluctuation and have a good regulation on blood glucose management.
