肝癌腹腔镜肝部分切除术后早期下床活动情况的研究

摘　　要：目的:探讨腹腔镜肝癌肝部分切除术后患者早期下床活动的情况。方法:纳入某医院肝脏外科住院的原发性肝癌患者为研究对象,随机分为试验组(25例)和对照组(24例),试验组行开腹肝部分切除术,对照组行腹腔镜肝部分切除术,术后采用相同的护理方法及下床活动指导,对比两组术后活动性疼痛程度、早期下床活动达标率、住院时间等差异。结果:两组患者术后48 h、72 h活动性疼痛评分差异有统计学意义(P<0.05),实验组患者术后首次下床活动时间、活动持续时间、活动步行距离、下床活动达标率、首次肛门排气、排便时间优于对照组(P<0.05)。结论:腹腔镜肝部分切除术有助于原发性肝癌患者早期下床活动,为将来进一步改善肝癌开腹肝部分切除患者术后康复护理提供参考。Objective:To investigate the early ambulation of patients after laparoscopic hepatectomy for hepatocellular carcinoma.Methods:Primary liver cancer patients hospitalized in the liver surgery department of hospital were included as the research objects,and were randomly divided into experimental group(n=25)and control group(n=24).The experimental group received open hepatectomy,and the control group received laparoscopic hepatectomy.The same nursing methods and out of bed movement guidance were used after surgery.The postoperative active pain degree,early ambulation rate and length of hospital stay were compared between the two groups.Results:There were statistically significant differences in pain scores between the two groups at 48 h and 72 h after surgery(P<0.05).The experimental group was superior to the control group in terms of first ambulation time,duration of ambulation time,ambulation distance,ambulation rate,first anal exhaust and defecation time(P<0.05).Conclusion:Laparoscopic hepatectomy can help patients with primary liver cancer to get out of bed early and provide reference for further improving postoperative rehabilitation nursing of patients with primary liver cancer after open hepatectomy.
