CASIA2与Pentacam测量高度近视前房深度、角膜厚度和瞳孔直径的对比研究

摘　　要：目的探讨应用CASIA2与Pentacam两种设备测量高度近视人群前房深度、角膜厚度和瞳孔直径的差异性和一致性。方法采用CASIA2与Pentacam两种设备分别测量48例18~43岁(24.52±6.90)岁高度近视人群的前房深度(ACD)、中央角膜厚度(CCT)和瞳孔直径,进行分析比较,并用Bland-Altman图来评估两种设备测量结果之间的一致性。结果CASIA2和Pentacam两种设备的前房深度(ACD)、中央角膜厚度(CCT)和瞳孔直径测量值之间差异均有统计学意义(P<0.05)。CASIA2所测前房深度和瞳孔直径值均大于Pentacam测量值,而CASIA2测得角膜厚度较Pentacam测量值小。Bland-Altman图得出CASIA2和Pentacam测得的中央角膜厚度和前房深度一致性较好,瞳孔直径在两种设备中的一致性较差。结论在高度近视人群中,CASIA2和Pentacam在测量角膜厚度和前房深度具有良好的一致性,两种设备可互换。但在临床特殊手术和一些研究中,也应注意两种设备的细微差异。Objective To compare the difference and consistency of anterior chamber depth(ACD),central corneal thickness(CCT)and pupil diameter measured by CASIA2 and Pentacam.Methods The central corneal thickness(CCT),anterior chamber depth(ACD)and pupil diameter of the right eye from forty-eight adults aged 18 to 43 years(24.52±6.90)years old were measured by CASIA2 and Pentacam,respectively.All subjects were measured twice with each device.The consistency between the measurement results of the two devices was evaluated by Bland Altman diagram.Results There were significant differences in anterior chamber depth(ACD),central corneal thickness(CCT)and pupil diameter between CAISA2 and Pentacam(P<0.05).The anterior chamber depth and pupil diameter measured by CAISA2 were greater than those measured by Pentacam,while the corneal thickness measured by CAISA2 was smaller than that measured by Pentacam.The Bland–Altman analysis suggested that the consistency of central corneal thickness and anterior chamber depth measured by Pentacam and CASIA2 was good,and the consistency of pupil diameter in the two devices was poor.Conclusions CASIA2 and Pentacam have good consistency in measuring corneal thickness and anterior chamber depth in people with high myopia.The two devices can be interchangeable.However,in clinical special surgery and some studies,we should also pay attention to the subtle differences between the two devices.
