以医院叙事伦理查房推动叙事医学落地的实践探索

摘　　要：叙事医学是循证医学发展到一定阶段的新需求、新拓展。以开展医院层面的叙事伦理查房,通过现场考察和实例解读案例导入叙事伦理理念、将理念内化为行为动力、鼓励引导医患主体间交流互动、反馈提升正向激励等方法,探索叙事医学临床实践新形式,在进一步促进医患和谐共创、克服职业倦怠、突破沟通困境、激发医务人员自我潜能释放和提高医院管理水平等方面取得了一定的成效。今后,将在叙事伦理查房机制建设和拓展与日常查房结合、考核评价指标以及问题协同解决机制、队伍建设等方面进一步探索。Narrative medicine is a new demand and expansion of evidence-based medicine to a certain stage.This article explores approaches such as hospital-level narrative ethics rounds,introducing narrative ethical concepts through on-site investigations and case interpretations,internalizing concepts as behavioral motivations,encouraging and guiding communication and interaction between doctors and patients,and feedback to enhance positive incentives.The new form of medical clinical practice has achieved certain results in further promoting the harmonious co-creation of doctors and patients,overcoming job burnout,breaking through communication difficulties,stimulating the release of medical staff's potentiality and improving hospital management.Further exploration will be made in the construction and expansion of the narrative ethical rounds mechanism and the integration of daily rounds,assessment and evaluation indicators,problem-solving mechanisms,and team building.
