摘 要:目的探究食管鳞状细胞癌(食管鳞癌)驱动蛋白超家族成员4A(KIF4A)、丝/苏氨酸激酶NIMA相关激酶2(NEK2)、细胞分裂周期蛋白20(CDC20)的表达及临床意义。方法选取2016年3月至2018年5月于邯郸市第一医院进行根治性手术治疗的91例食管鳞癌患者,术中取癌组织和癌旁组织(距癌组织3 cm),采用免疫组化法和实时荧光定量PCR法检测不同组织中KIF4A、NEK2和CDC20的表达,分析三者表达水平与患者临床病理特征及预后的关系。结果癌组织中KIF4A、NEK2和CDC20mRNA水平及蛋白表达评分均高于癌旁组织,差异具有统计学意义(P<0.05)。不同临床分期和淋巴结转移情况的KIF4A表达比较,差异有统计学意义(P<0.05),不同临床分期、分化程度和淋巴结转移情况的NEK2和CDC20表达比较,差异有统计学意义(P<0.05)。随访3年,Kaplan-Meier生存曲线结果表明,KIF4A、NEK2和CDC20阳性表达者生存时间分别短于KIF4A、NEK2和CDC20阴性表达者,差异有统计学意义(P<0.05)。Cox回归分析显示,低分化程度、存在淋巴结转移及CDC20阳性表达是影响食管鳞癌患者预后存活的独立危险因素(P<0.05)。结论食管鳞癌患者癌组织中KIF4A、NEK2和CDC20阳性表达率异常升高,可为食管鳞癌病理状态和预后判定提供参考。Objective To explore the expressions and clinical significance of kinesin family member 4 A(KIF4 A),NIMA,Never-in-mitosis A-related kinase 2(NEK2)and cell division cyclin 20(CDC20)in esophageal squamous cell carcinoma(ESCC). Methods A total of 91 patients with ESCC undergoing radical surgery in Handan First Hospital were enrolled from March 2016 to May 2018. The carcinoma tissues and paracarcinoma tissues(3 cm away from carcinoma tissues)were collected during the surgery. The expressions of KIF4 A,NEK2 and CDC20 in different tissues were detected by immunohistochemistry and real-time fluorescence quantitative PCR. And their relationship with clinicopathological characteristics and prognosis of ESCC patients was analyzed. Results The levels of KIF4 A,NEK2 and CDC20 mRNA and proteins in carcinoma tissues were higher than those in para-carcinoma tissues(P<0.05). The differences in expression of KIF4 A in patients with different clinical staging and lymph node metastasis were statistically significant(P<0.05),and differences in expressions of NEK2 and CDC20 in patients with different clinical staging,differentiation degree and lymph node metastasis were statistically significant(P<0.05). After 3 years of follow-up,Kaplan-Meier survival curve results showed that the survival time in patients with positive expressions of KIF4 A,NEK2 andCDC20 was shorter than that with negative expressions(P<0.05). Cox regression analysis showed that the degree of poor differentiation,the presence of lymph node metastasis and the positive expression of CDC20 were independent risk factors affecting the prognosis and survival of patients with esophageal squamous cell carcinoma(P<0.05). Conclusion The positive expression rates of KIF4 A,NEK2 and CDC20 in carcinoma tissues is abnormally increased in patients with ESCC,which can provide a reference for determining the pathological state and prognosis of ESCC.
