贝复舒凝胶联合典必殊眼膏及明胶海绵对鼻内镜下泪囊鼻腔吻合术伤口愈合的疗效  

Effect of Beifushu Gel Combined with Dianbishu Ointment and Gelatin Sponge on Wound Healing after Dacryocystorhinostomy under Nasal Endoscopy

摘　　要：目的探讨贝复舒凝胶联合典必殊眼膏对泪囊鼻腔吻合口愈合的疗效。方法收集90例单眼慢性泪囊炎病例,随机分成贝复舒组、典必殊组和联合组,每组30例。所有患者均由同一名经验丰富的眼眶外科医师在鼻内镜下操作完成泪囊鼻腔吻合术。贝复舒组在吻合口及周围贴敷浸有贝复舒凝胶的明胶海绵;典必殊组在吻合口及周围贴敷浸有典必殊眼膏的明胶海绵;联合组在吻合口及周围贴敷浸有2种药物的明胶海绵。术后1个月于鼻内镜下观察各组患者吻合口愈合情况;术后3个月询问患者溢泪、溢脓症状并行泪道冲洗以评估各组的疗效。结果术后1个月,典必殊组吻合口痊愈21例,愈合不佳6例,愈合失效3例;贝复舒组痊愈23例,愈合不佳5例,愈合失效2例;联合组痊愈28例,愈合不佳2例。联合组吻合口愈合情况优于典必殊组(P<0.05);贝复舒组吻合口愈合情况与典必殊组及联合组比较,差异均无统计学意义(P>0.05)。术后3个月,典必殊组、贝复舒组和联合组有效率分别为90.00%、93.30%和100.00%,3组有效率比较差异无统计学意义(P>0.05)。结论贝复舒凝胶联合典必殊眼膏及明胶海绵较典必殊眼膏及明胶海绵更能促进泪囊鼻腔吻合术吻合口的愈合。Objective To explore the effect of Beifushu gel combined with Dianbishu ointment on wound healing after dacryocystorhinostomy.Methods Ninety cases of monocular chronic dacryocystitis were randomly divide into three groups,with 30 cases in each group.Dacryocystorhinostomy was performed under nasal endoscopy by an experienced orbital surgeon.Gelatin sponges impregnated with Beifushu gel,Dianbishu ointment and their combination were applied to the anastomosis and surrounding area in Dianbishu group,Beifushu group and combined group,respectively.One month after operation,anastomotic healing was observed under nasal endoscopy.Three months after the operation,the patients were asked about discharge of tears and pus and lacrimal duct irrigation to evaluate the curative effect.Results One month after operation,respectively,21,23 and 28 patients achieved complete anastomotic healing,6,5 and 2 patients had poor healing,and 3,2 and 0 patients failed to heal in Dianbishu group,Beifushu group and combined group.The anastomotic healing in combined group was better than that in Dianbishu group(P<0.05);no significant difference in anastomotic healing was found between beifushu group and dianbishu group,so was between beifushu group and combined group(P>0.05).At 3 months after the operation,no significant difference in the effective rate was found among Dianbishu group(90.00%),Beifushu group(93.30%)and combined group(100.00%)(P>0.05).Conclusion Beifushu gel combined with Dianbishu ointment and gelatin sponge can promote wound healing after dacryocystorhinostomy compared with Dianbishu ointment and gelatin sponge.
