摘 要:目的观察二氧化锆全瓷冠在全冠口腔修复患者中的临床效果及其对龈沟出血、咀嚼功能的影响。方法将113例需行全冠口腔修复的患者按修复材料不同分为镍铬合金组56例和二氧化锆组57例。镍铬合金组行镍铬合金烤瓷冠修复,二氧化锆组行二氧化锆全瓷冠修复。比较2组治疗3个月后牙龈颜色匹配度、牙龈边缘密合度及牙龈修复完整度,观察2组治疗前后龈沟出血[包括牙龈出血指数(SBI)、牙龈龈沟深度探诊(GCD)]、咀嚼功能[包括咬合力、咀嚼效率及牙齿动度值(PTV)]变化情况。结果二氧化锆组治疗3个月后的牙龈颜色匹配度、牙龈边缘密合度及牙龈修复完整度均显著高于镍铬合金组(P<0.05)。2组治疗后SBI、GCD值均较治疗前显著升高,但二氧化锆组升高幅度显著低于镍铬合金组,差异均有统计学意义(P<0.001)。2组治疗后咬合力、咀嚼效率较治疗前显著升高,PTV值较治疗前显著降低,且二氧化锆组变化幅度显著高于镍铬合金组,差异均有统计学意义(P<0.001)。结论二氧化锆全瓷冠可通过良好的组织相容性提高全冠口腔修复患者的牙齿贴合度,改善龈沟出血状态,调节修复效果,提高咀嚼能力。Objective To investigate the effects of zirconium dioxide all-ceramic crown on gingival sulcus bleeding and masticatory ability in patients undergoing full-crown restoration.Methods According to different restoration materials,113 patients who needed full crown prosthesis were repaired with either Ni-Cr alloy porcelain crown(56 patients)or zirconium dioxide all-ceramic crown(57 patients).Gingival color matching,margin fitness and repair integrity were compared between the two groups after treatment for 3 months.Furthermore,sulcus bleeding index(SBI),gingival crevicular depth(GCD),occlusal force,chewing efficiency and periotest value(PTV)were observed before and after treatment.Results After 3 months of treatment,gingival color matching degree,margin fitness and repair integrity in zirconium dioxide group were higher than those in Ni-Cr alloy group(P<0.05).After treatment,SBI and GCD in 2 groups were significantly higher than those before treatment,but the increase in zirconium dioxide group was significantly lower than that of Ni-Cr alloy group(P<0.001).After treatment,the occlusal force and masticatory efficiency in 2 groups were significantly increased than those before treatment,and the value of PTV in 2 groups was significantly lower than that before treatment,and the change range of zirconium dioxide group was significantly higher than that of Ni-Cr alloy group(P<0.001).Conclusion Zirconium dioxide all-ceramic crown has good histocompatibility,which can improve tooth fitness,ameliorate gingival sulcus bleeding,adjust repair effect,and enhance masticatory ability in patients undergoing full-crown restoration.
