盐酸米诺环素软膏联合热牙胶根管治疗重度牙周-牙髓病变患者的效果

摘　　要：目的分析重度牙周-牙髓病变特点、治疗要点,研究盐酸米诺环素软膏联合热牙胶根管治疗的优势。方法选择我院口腔科2019年3月至2020年6月收治的80例重度牙周-牙髓病变患者,进行随机法分组治疗。两组均予以牙周基础治疗,对照组行热牙胶根管治疗,观察组联合盐酸米诺环素软膏治疗。比较两组患者的治疗效果、不良反应发生情况以及治疗后2个月的牙周探诊深度(PD)、菌斑指数(PLI)、牙龈指数(GI)水平。结果治疗前,两组PD、PLI、GI水平比较,差异无统计学意义(P>0.05);治疗后,两组PD、PLI、GI水平均较治疗前降低,观察组低于对照组(P<0.05);观察组治疗总有效率高于对照组(P<0.05);观察组不良反应发生率与对照组接近(P>0.05)。结论盐酸米诺环素软膏、热牙胶根管联合治疗重度牙周-牙髓病变患者的效果理想,有助于患者牙周指标的改善,且安全性获得保障。Objective To analyze the characteristics and treatment points of severe periodontal and pulpal lesions,and to study the advantages of minocycline hydrochloride ointment combined with hot gutta-percha root canal treatment.Methods Select 80 patients with severe periodontalendodontic disease admitted in the Department of Stomatology of our hospital from March 2019 to June 2020 for randomized treatment.In addition to basic periodontal treatment,the control group received hot gutta-percha root canal treatment,and the observation group was treated with minocycline hydrochloride ointment.The effects and adverse reactions after treatment of the two groups of patients were compared,and the probing depth(PD),plaque index(PLI),and gingival index(GI)levels of patients 2 months after treatment were compared.Results Comparing the levels of relevant indicators,the levels of PD,PLI,and GI in the two groups were lower than before treatment,and the observation group was lower than the control group,P<0.05.The treatment effect and adverse reactions were compared,and the total effective rate of the observation group washigher than control group(P<0.05),and the adverse reaction rate was close to that of the control group(P>0.05).Conclusion The combination of minocycline hydrochloride ointment and hot gutta-percha root canal has an ideal effect in the treatment of severe periodontal-pulp lesions,which can help improve the periodontal indexes of patients,and the safety is guaranteed.
