摘  要:目的探讨经支气管镜辅助灌注药物治疗难治性空洞型肺结核的临床效果,以期为临床治疗该病提供参考。方法回顾性选取2019年2月至2020年2月本院100例难治性空洞型肺结核患者,根据治疗方法将其分为单纯性化疗组和经支气管镜组,各50例。单纯性化疗组给予2HRZS(E)/4HR方案治疗,经支气管镜组在单纯性化疗组基础上给予经支气管镜辅助灌注药物治疗。比较两组的痰菌转阴、病灶吸收、空洞闭合情况及毒副反应发生情况。结果经支气管镜组的痰菌转阴率、病灶显著吸收率高于单纯性化疗组,病灶不变率、病灶恶化率低于单纯性化疗组,差异具有统计学意义(P<0.05)。经支气管镜组的空洞闭合率高于单纯性化疗组,差异具有统计学意义(P<0.05)。经支气管镜组的毒副反应总发生率为6.00%,单纯性化疗组的毒副反应总发生率为6.00%,组间比较,差异无统计学意义(P>0.05)。结论经支气管镜辅助灌注药物治疗难治性空洞型肺结核的效果较好,可促使痰菌转阴、病灶吸收、部分空洞闭合或空洞明显缩小,且不增加毒副反应,安全性较好。Objective To investigate the clinical effect of transbronchoscopy-assisted perfusion medication in the treatment of refractory cavitary pulmonary tuberculosis,in order to provide reference for clinical treatment of the disease.Methods One hundred patients with refractory cavitary pulmonary tuberculosis in our hospital from February 2019 to February 2020 were retrospectively selected.According to the treatment methods,the patients were divided into simple chemotherapy group and transbronchoscopy group,with 50 cases in each group.The simple chemotherapy group was treated with 2HRZS(E)/4HR regimen,and the transbronchoscopy group was treated with transbronchoscopy-assisted perfusion medication on the basis of the simple chemotherapy group.The sputum bacteria turned negative,lesion absorption,cavity closure and occurrence of toxic and side effect were compared between the two groups.Results The sputum bacteria turned negative rate and significant absorption rate of lesions in the transbronchoscopy group were higher than those in the simple chemotherapy group,and the lesion unchanged rate and lesion deterioration rate were lower than those in the simple chemotherapy group,the differences were statistically significant(P<0.05).The cavity closure rate in the transbronchoscopy group was higher than that in the simple chemotherapy group,and the difference was statistically significant(P<0.05).The total incidence of toxic and side effect in the transbronchoscopy group was 6.00%,and that in the simple chemotherapy group was 6.00%,there was no significant difference between the groups(P>0.05).Conclusion Transbronchoscopy-assisted perfusion medication in the treatment of refractory cavitary pulmonary tuberculosis has a better effect.It can promote sputum bacteria turned negative,lesion absorption,part of cavity closure or the cavity reduced significantly,without increasing toxic and side effect and has good safety.
