邯郸市3~5岁儿童口腔健康临床指数和生活质量的相关性分析

摘　　要：目的探讨邯郸市3~5岁儿童口腔健康临床指数与生活质量的相关性。方法采用多阶段、整群、随机抽样的方法,在邯郸市区抽取383名学龄前儿童进行口腔检查和临床指数的判定,并进行口腔健康相关生活质量调查和相关性分析。结果 372名儿童符合纳入标准,应答率为97.13%。372名儿童的患龋率为69.35%,SECC患病率为55.65%,错牙合畸形患病率为40.05%,色素沉着患病率为11.29%,软垢指数为1.15±0.71。邯郸市3~5儿童的口腔健康影响程度量表(C-ECOHIS)平均分为13.38±6.56,因口腔问题出现"疼痛"的频率最高,因口腔问题影响最小的是"对家庭造成经济负担"。经统计学分析分数有显著性差异的因素有不同龋齿状态、有无色素沉着、不同牙合关系类型、不同年龄,父亲学历、母亲学历和家庭月收入(P <0.05)。多重线性回归分析结果显示与生活质量有相关性的因素有软垢指数、龋齿状态、色素沉着(P <0.05)。结论邯郸市3~5岁儿童口腔健康状况较差,软垢指数、龋齿状态、色素沉着与生活质量存在一定的相关性。针对相关因素采取干预措施对改善该地区3~5岁儿童口腔健康状况,提高其生活质量具有重要意义。Objective To investigate the correlation between oral health clinical index and quality of life of children aged 3 to 5 years in Handan.Methods A total 383 preschool children in Handan city were selected through multi-stage,cluster and random sampling method for oral examination and oral health-related quality of life survey.Results A total of 372 children met the inclusion criteria and the response rate was 97.13%.The prevalence of caries,SECC,malocclusion,black stain and debris index-simplified were 69.35%,55.65%,40.05%,11.29%and 1.15±0.71,respectively.The score of the early childhood oral health impact scale(ECOHIS)for children was 13.38±6.56.The highest frequency due to oral problems was pain,and the least impact of oral problems was economic burden on the family.The factors that had significant differences in scores included different caries status,presence or absence of black stain,different types of occlusion,different ages,father’s and mother’s education and monthly family income(P<0.05).The factors related to quality of life were debris index-simplified,caries status and black stain(P<0.05).Conclusions The oral health status of children aged 3 to 5 in Handan is poor,and there is a certain correlation between the index-simplified,caries status,black stain and quality of life.
