摘  要:目的:探讨肝脏硬度值联合HBV DNA水平在慢性HBV感染者肝纤维化的诊断效能。方法:回顾分析2017年1月至2018年12月就诊的慢性HBV感染者210例,根据肝活检病理结果,将患者分为无纤维化组、轻度组、中度组、重度组。运用受试者工作特征曲线(ROC)评估FibroScan检测肝硬度(LSM)联合HBV DNA水平在慢性HBV感染者肝纤维化中的诊断效能。结果:中度组和重度组患者的LSM显著升高(P<0.05),HBV DNA定量显著降低(P<0.05)。LSM联合HBV DNA对诊断无纤维化,轻度、中度、重度纤维化ROC曲线AUC分别为0.850、0.648、0.652和0.954。结论:FibroScan联合HBV DNA是评估慢性HBV感染患者肝纤维化的有效工具,对筛查慢性HBV感染者肝纤维化有较好价值。Objective:To investigate the liver stiffness(LSM)combined with HBV DNA in the diagnosis of liver fibrosis in patients with chronic HBV infection.Methods:A total of 210 patients with chronic HBV infection from January 2017 to December 2018 were retrospectively analyzed.According to the pathological results of liver biopsy,the patients were divided into no fibrosis group,mild group,moderate group and severe group.Receiver operating characteristic curve(ROC curve)was used to evaluate the diagnostic efficacy of LSM combined with HBV DNA level in patients with chronic HBV infection.Results:LSM in moderate group and severe group was significantly increased(P<0.05).HBV DNA quantitation in moderate and severe groups was significantly decreased(P<0.05).The AUC of the ROC curve of LSM combined with HBV DNA in the diagnosis of non fibrosis,mild,moderate and severe fibrosis were 0.850,0.648,0.652 and 0.954,respectively.Conclusion:FibroScan combined with HBV DNA is an effective tool to evaluate liver fibrosis in patients with chronic HBV infection,and has a good value in screening liver fibrosis in patients with chronic HBV infection.
