内镜下切除结肠巨大息肉的临床经验探讨

摘　　要：目的分析内镜下切除结肠巨大息肉的有效性及安全性。方法回顾性分析53例因结肠巨大息肉(≥2.0 cm)行内镜下切除的患者的临床资料,就内镜的切除方式、切除时间、止血方式、术后病理、术后并发症及随访情况进行分析。结果结肠巨大息肉主要分布于直乙结肠,以山田Ⅳ型最常见;组织学上以腺瘤性息肉多见。所有息肉均成功切除,手术时间短。仅1例出现术中出血,无术后出血及穿孔病例。术后1例转外科行根治手术,其余患者均规律随访,未见复发。结论内镜下切除结肠巨大息肉是一种安全、可靠、成功率高的方法。Objective To analyze the efficacy and safety of endoscopic resection of giant colonic polyps.Methods The clinical data of 53 patients who underwent endoscopic polypectomy for giant colonic polyps(≥2.0 cm)were reviewed,the methods of endoscopic resection,resection time,hemostasis method,postoperative pathology,postoperative complications and follow-up were analyzed.Results Giant colonic polyps were mainly distributed in the rectum and sigmoid colon.Yamada type IV was the most common type.Adenomatous polyps were most common histologically.All polyps were successfully resected,and the resection time was short.Only one case had intraoperative bleeding,and there was no case of postoperative bleeding and perforation.After operation,1 case was transferred to surgery for radical operation.The other patients were followed up regularly and no recurrence was found.Conclusions Endoscopic resection of giant colonic polyps is a safe,reliable and successful method.
