三种镍钛根管预备器械对重度弯曲根管的成形能力比较

摘  要:目的:比较三种镍钛根管预备器械对重度弯曲根管的成形能力。方法:选取因重度牙周病而拔除的120颗伴有近中根管重度弯曲的下颌磨牙作为研究对象,按照随机数字表法分为A组、B组和C组各40颗离体牙。分别对三组离体牙应用不同镍钛器械做根管预备处理,A组应用ProTaper Next(PTN)锉,B组应用Twisted File Adaptive(TFA)锉,C组应用Reciproc(Rec)锉,比较三组预备前后根管弯曲度、根尖偏移指数(ATI)、根管预备时间和根管侧壁穿孔发生率。结果:三组预备前后根管弯曲度组间比较,差异无统计学意义(P>0.05);三组ATI和根管预备时间比较,差异均有统计学意义(P<0.05),其中A组、B组ATI均低于C组,A组、B组根管预备时间均长于C组,差异有统计学意义(P<0.05);A组、B组ATI和根管预备时间比较,差异均无统计学意义(P>0.05);三组根管侧壁穿孔发生率比较,差异无统计学意义(P>0.05)。结论:三种镍钛根管预备器械对重度弯曲根管的成形效果各异,其中PTN锉、TFA锉能减少根尖偏移、保持原始解剖结构,Rec锉能缩短根管预备时间,临床应根据实际情况合理选择相应的根管预备器械。Objective:To compare shaping abilities of three kinds of nickel-titanium root canal preparation instruments for severely curved root canals.Methods:120 mandibular molars with severe curved mesial root canal,which were extracted due to severe periodontal disease,were selected as the research objects.According to the random number table method,they were divided into groups A,B and C,each with 40 extracted teeth.The three groups were treated with different nickel-titanium instruments for root canal preparation.Group A used ProTaper Next(PTN)files,group B used Twisted File Adaptive(TFA)files,and Group C used Reciproc(Rec)files.The root canal curvature,the apical transportation index(ATI),the root canal preparation time and the incidence of root canal lateral wall perforation were compared among the three groups before and after the preparation.Results:There were no significant differences in the root canal curvature among the three groups before and after the preparation(P>0.05).There were statistical differences in the root canal preparation time among the three groups.The ATIs of group A and group B were lower than that of group C;the root canal preparation times of group A and group B were longer than that of group C,and the differences were statistically significant(P<0.05).There were no significant differences in the ATI and the root canal preparation time between group A and group B(P>0.05).Further,there were no statistically significant differences in the incidence of side wall perforation among the three groups(P>0.05).Conclusions:Three kinds of nickel-titanium root canal preparation instruments have different shaping effects on the severely curved root canals.Among them,PTN files and TFA files can reduce the root apex transportation and maintain the original anatomical structure,and Rec files can shorten the root canal preparation time.Therefore,the clinic should reasonably choose the corresponding root canal preparation instruments according to the actual situation.
