摘　　要：目的:探讨腹腔镜辅助治疗横、降结肠癌的短期与长期疗效及安全性。方法:纳入2012年5月至2016年5月收治的需手术治疗的68例横结肠或降结肠癌患者,随机分配至腹腔镜辅助结肠切除术(LAC)组与传统开放结肠切除术(OC)组,每组34例。记录两组围手术期相关手术指标及术后并发症发生率,随访并记录5年无复发生存率、5年总生存率。结果:LAC组与OC组手术时间[(197.4±31.5)min vs.(150.0±42.8)min,P<0.001]、术中出血量[(111.0±47.1)mL vs.(176.8±36.1)mL,P<0.001]、住院时间[(8.3±0.9)d vs.(10.0±1.1)d,P<0.001]差异有统计学意义;两组术后并发症发生率、5年无复发生存率、总生存率差异无统计学意义(P>0.05)。结论:LAC治疗横结肠与降结肠癌的长期疗效与OC相近,但LAC的短期疗效优于开放手术。Objective:To investigate the short-term and long-term efficacy and safety of laparoscopic assisted colectomy(LAC)for transverse and descending colon cancer.Methods:From May 2012 to May 2016,68 patients with transverse or descending colon cancer who required surgical treatment were enrolled.Patients were randomly assigned to LAC group and open colectomy(OC)group with 34 cases in each group.Perioperative operative indicators and incidence of postoperative complications were recorded in 2 groups.Patients were followed up and 5-year recurrence-free survival and 5-year overall survival were recorded.Results:There were statistically significant difference in the operative time[(197.4±31.5)min vs.(150.0±42.8)min,P<0.001],the intraoperative blood loss[(111.0±47.1)mL vs.(176.8±36.1)mL,P<0.001],the mean hospital stay[(8.3±0.9)d vs.(10.0±1.1)d,P<0.001]between LAC group and OC group.There was no significant difference in the incidence of postoperative complications,5-year recurrence-free survival and 5-year overall survival between the two groups(P>0.05).Conclusions:The long-term efficacy of LAC in the treatment of transverse and descending colon cancer is similar to OC.In terms of short-term efficacy,LAC is a better choice than open surgery.
