隐形矫治器在青少年安氏Ⅱ类错[牙合]畸形治疗中的应用

摘　　要：目的探讨在青少年安氏Ⅱ类错[牙合]畸形治疗中应用隐形矫治器的效果。方法选取2018年1月至2020年12月在虞城县人民医院口腔科就诊的50例青少年安氏Ⅱ类错[牙合]畸形患者作为研究对象,依据随机数表法分为对照组和观察组,各25例。对照组采用固定矫治器治疗,观察组采用隐形矫治器治疗。于矫治6个月时,对比两组的矫治效果[美国正畸协会(ABO)评价系统]、满意度以及矫治前和矫治6个月时的牙周健康情况[菌斑指数(PLI)、牙龈指数(GI)、探诊深度(PD)];比较两组矫治6个月的并发症发生率。结果两组矫治6个月的ABO评价系统各项目评分对比,差异无统计学意义(P>0.05);矫治6个月,两组PLI、GI评分高于矫治前,PD深于矫治前,观察组PLI、GI评分低于对照组,PD浅于对照组,差异有统计学意义(P<0.05);观察组美观度、咀嚼功能、舒适度评分均高于对照组,差异有统计学意义(P<0.05)。观察组矫治6个月期间的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论在青少年安氏Ⅱ类错[牙合]畸形治疗中应用隐形矫治器的效果与固定矫治器相当,但隐形矫治器更利于保护患者的牙周健康,并发症发生率更低,患者对隐形矫治器的满意度更高。Objective To investigate the effect of invisible appliance in the treatment of Angle classⅡmalocclusion in adolescents.Methods Fifty children with Angle classⅡmalocclusion admitted to Department of Stomatology,Yucheng County People’s Hospital from January 2018 to December 2020 were selected as the research subjects.According to the random number table method,they were divided into control group and observation group,with 25 cases in each group.The control group was treated with fixed appliance,and the observation group was treated with invisible appliance.Six months after correction,the orthodontic effect[American orthodontic Association(ABO)evaluation system],satisfaction degree and periodontal health before and 6 months after correction[plaque index(PLI),gingival index(GI),probing depth(PD)]of the two groups were compared.The incidence rates of complications after 6 months of correction were compared between the two groups.Results There was no statistical difference in the scores of each item of ABO evaluation system between the two groups after 6 months of correction(P>0.05).After 6 months of correction,PLI and GI scores of both groups were higher than those before correction,and PD was deeper than that before treatment,and the PLI and GI scores of observation group were lower than those of control group,and the PD was shallower than that of control group(P<0.05).The scores of aesthetics,masticatory function and comfort in observation group were higher than those in control group(P<0.05).The total incidence of complications after 6 months of correction in observation group was lower than that in control group(P<0.05).Conclusion The effect of invisible appliance is similar to that of fixed appliance in the treatment of Angle classⅡmalocclusion,however,the invisible appliance is more conducive to protect patients’periodontal health,the incidence of complications is lower,and patients are more satisfied with the invisible appliance.
