摘 要:目的探讨急性髓系白血病(AML)患者血清微小RNA-182-5p(miR-182-5p)和同源盒基因A9(HOXA9)信使核糖核酸(mRNA)的表达水平,并分析二者与AML治疗后复发的关系。方法选取2018年4月—2020年3月四川大学华西医院血液内科收治的AML患者110例作为研究对象,根据患者治疗后1年内是否复发,分为AML缓解组(75例,治疗后1年内未复发)和AML复发组(35例,治疗后1年内复发)。另选取同期在医院体检健康者50例作为健康对照组。收集受试者实验室指标,实时荧光定量PCR法检测血清miR-182-5p和HOXA9 mRNA表达水平,分析血清miR-182-5p、HOXA9 mRNA表达水平及其相关性,AML患者复发的影响因素及血清miR-182-5p和HOXA9 mRNA表达水平对AML复发的预测价值。结果健康对照组、AML缓解组、AML复发组初诊时WBC水平、骨髓原始细胞比例依次升高,Hb、PLT水平依次降低(F/P=139.273/0.000、150.277/0.000、136.994/0.000、179.436/0.000);健康对照组、AML缓解组、AML复发组血清miR-182-5p、HOXA9 mRNA表达水平依次升高(F/P=254.519/0.000、129.910/0.000);血清miR-182-5p与HOXA9 mRNA表达水平呈正相关(r/P=0.519/0.000);血清miR-182-5p、HOXA9 mRNA表达水平高是影响AML患者复发的危险因素[OR(95%CI)=2.453(1.939~3.103)、2.341(1.524~3.596),P=0.000];血清miR-182-5p、HOXA9 mRNA预测AML复发的曲线下面积(AUC)为0.741、0.836,二者联合检测预测AML复发的AUC为0.861,优于单独预测者(Z/P=2.919/0.004、1.682/0.046)。结论血清miR-182-5p和HOXA9 mRNA可能通过高表达参与AML复发的进程,二者对AML复发的诊断可能有重要意义。Objective To explore the expression levels of serum microRNA-182-5p(miR-182-5p)and homeobox gene A9(HOXA9)messenger ribonucleic acid(mRNA)in patients with acute myeloid leukemia(AML),and analyze the relationship between the two and relapse after AML treatment.Methods From April 2018 to March 2020,110 AML patients admitted to the Department of Hematology,West China Hospital,Sichuan University were selected as the research objects.According to whether the patients relapsed within 1 year after treatment,they were divided into AML remission group(75 cases,no recurrence within 1 year after treatment)and AML recurrence group(35 cases,recurrence within 1 year after treatment).In addition,50 healthy people who had a physical examination in the hospital during the same period were selected as the healthy control group.Collect subjects'laboratory indicators,detect serum miR-182-5p and HOXA9 mRNA expression levels by real-time fluorescent quantitative PCR,analyze serum miR-182-5p,HOXA9 mRNA expression levels and their correlation,influencing factors of relapse in AML patients,and serum miR-182-5p and HOXA9 mRNA The predictive value of expression level for AML recurrence.Results In the healthy control group,AML remission group,and AML relapse group,WBC levels and the proportion of bone marrow blasts increased in sequence at the first diagnosis,and Hb and PLT levels decreased in sequence(F/P=139.273/0.000,150.277/0.000,136.994/0.000,179.436/0.000).The expression levels of serum miR-182-5p and HOXA9 mRNA in the healthy control group,AML remission group,and AML relapse group increased sequentially(F/P=254.519/0.000,129.910/0.000).There was a positive correlation between serum miR-182-5p and HOXA9 mRNA expression level(r/P=0.519/0.000).High serum miR-182-5p and HOXA9 mRNA expression levels are risk factors affecting the recurrence of AML patients[OR(95%CI)=2.453(1.939-3.103),2.341(1.524-3.596),P=0.000].The area under the curve(AUC)of serum miR-182-5p and HOXA9 mRNA predicting AML recurrence is 0.741 and 0.836.The combined detect
